    Job summary

    An exciting opportunity has arisen for a communityin-reach Alcohol Liaison Nurse to work aspart of the Alcohol CareTeam at Southmead Hospital (run by North Bristol NHS Trust).

    The post holder will be required to be a Registered Generalnurse with substantive experience of working in a substance misuse team and/orspecific alcohol team experience in a community setting. Experience in workingwith multi-disciplinary staff of all levels and developing services would be anadvantage. The post is alink role to ensure wrap around care back into the community for patients ondischarge.

    Main duties of the job

    To workas an integral part of Southmead Hospitals Alcohol Care Team with a role in symptommanagement, care planning, and in-patient and discharge support for patients underthe team's care.

    The post-holder willdiscuss relapse prevention with stabilised patients within the hospital andsupport them to access the community-based structured treatment services.

    The post-holder will work as part of the hospitals drug andalcohol team, ensuring that ED and ward staff are aware of alcohol relatedissues and are committed to screening, identifying, and referring patients whoare dependent or high-risk drinkers.

    The post-holder will be employed by Hanham Health, providerof complex clinical care to drinkers in the community,butwill be line managed by the hospital's Lead Nurse for Addictions in order to be fully integrated with the hospitalteam.

    The post-holder will act as a liaison between the hospitaland the community drug and alcohol service, ensuring patients are proactively and dynamically managed between services.

    The aim of this role is to bridge the gap between hospital and communityservices and to strengthen the relationship and collaboration between the two.

    Job description

    Job responsibilities

    The post holder will use their knowledge, whichis underpinned by theory, evidence, training, and experience, to implementclinical, and process policy/protocols, which consolidate and enhance safeworking practices.

    The post holder will promote to hospital staffany education and training programmes being offered by Hanham Health in themanagement of substance misuse.

    The post holder will propose service changeswhich impact on their own specialist area.

    The post holder will, to a high standard, support pathways of careand provide specialised advice concerning that care to professionals, patients,and relatives/carers.

    The post holder will use their knowledge tomanage situations which require analysis, interpretation, and comparison of a rangeof options after assessing specialist acute and other conditions found inclients who are experiencing multi-diagnosis of alcohol misuse with medicalcomplications.

    The post holder will operate in line with theNMC Code of Professional Conduct and other relevant ethical Codes of Practice.

    The post holder will act within the guidance,policies and procedures that are issued by Hanham Health and NBT and complywith Mental Health, NBTworking practice and other legislation, as appropriate.

    The post holder will maintain and continuallyimprove their clinical practice.

    The post holder will:

    o Be part of a team that developsplans of care for patients who are screened by A&E and/or wardpatients for alcohol and drug use within the concept of the model of harm minimisation.

    oProvide clinical advice on the management ofclients with alcohol dependence or other alcohol problems to general medicalteams, within their sphere of competence and knowledge with guidance and support from the NBT AlcoholCare Team (ACT).

    oProvide clinical advice on alcohol treatmentoptions including medication-prescribing regimes to ED/ward medical and nursingstaff, within their sphere of competence and knowledge. Following NBT alcoholassessment guidelines and pathways

    o Assessclients who are allocate tothem by the ACT in the hospital.

    o Assessthe psychological, emotional, social and treatment needs of the client, takingaccount of the needs of relatives/carers and local community.

    o Plan,deliver and evaluate treatment interventions as clinically appropriate, usingcurrent evidence.

    o Providebrief interventions, health education, advice, and information to clientsregarding their alcohol use and inform them of local services available tothem.

    o Manageand care co-ordinate clients with complex needs and/or high-risk levels untiltransfer to core specialist services.

    o Implementlocal Clinical Risk Assessment and Management Protocols.

    o Appropriatelyrefer clients as part of care planned agreements to other statutory andnon-statutory services for longer-term medical and psychosocial interventionssuch as detoxification, counselling, psychology, and family therapy.

    o Attendand remain up to date with supervision and training, as appropriate to therole.

    Qualifications

    Essential

  • Registered Nurse RGN
  • Qualification or keen interest in Drug and Alcohol/Addiction
  • Brief intervention, solution focused or similar counselling training & Motivational Interviewing
  • Desirable

  • Experience of working in an Acute setting
  • Knowledge, Skills and Attributes

  • Ability to work well within an integrated, Hospital based, drug and alcohol team
  • Ability to work creatively to find patient centred solutions
  • Ability to deliver services within a multi-disciplinary setting
  • Demonstrate excellent interpersonal skills in varied settings
  • Ability to work with a wide range of partners and professionals
  • Knowledge of current legislation and initiatives in alcohol, substance misuse and accident and emergency medicine
  • Understanding of the competencies associated with an alcohol care team and knowledge of managing alcohol withdrawal using both clinically indicated withdrawal assessments and standard reducing regimens
  • Knowledge of the legal and ethical issues relating to substance misuse and health care
  • Be able to demonstrate flexibility
  • Adherence to local policy and procedure in line with government strategies and recommendations
