All Age Continuing Care (AACC) haveopportunities for band 6 Senior Practitioners to join our Clinical Bank.

The post holder will play a key role across the CHC service,working within the remit of the National Framework for Continuing Healthcare,to deliver an effective, efficient and equitable service to the people andpopulations of Sussex.

Prior experience of CHC would be beneficial, but is notessential for this role, as you will work within an experienced and supportiveteam who will support you to develop the relevant knowledge required. The postholder should however have experience of working within health or care, andwill need excellent verbal and written communication skills, as you will berequired to work closely with patients, families, carers and other externalpartners.

Main duties of the job

As a Senior Practitioner, your role will include delivery of avariety of functions across the CHC service, including: Assessment, Reviews,Care Planning and Case Management.

You will also have the opportunity to work across other areas ofCHC, including audit, appeals, retrospective assessments, complaints,assessment verification, and many more.

You will be supported to develop the necessary skills andexperience to work across the whole pathway, safely, confidently andeffectively.

Job responsibilities

You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. The role will require travel on occasions, to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public. All CHC staff are expected to work flexibly, when required, to meet the needs of the service.
