An exciting opportunity has arisen for an experienced Band 4 Healthcare Assistant (HCA) preferably with some experience in Cardiology or Heart failure to develop and enhance their skills within the community setting as part of a proactive and forward-thinking team.


You will work as part of an established community heart failure service providing support to patients and carers within their home environment and / or clinic setting, under the direction of the Community Heart Failure Nurse Specialist.

This includes assisting in the ongoing assessment and provision of care and reporting changes in patient status to the nurses.

The role will involve promoting optimum independence and individualised patient centred care.

The post-holder will be joining a multi-disciplinary team working with other Heart Failure Specialist Nurses, Cardiologists, and the wider Brent Integrated service to support patients who are at risk of admissions to hospital.


The role of the HCA is to work with the multi professional team and the Registered Nurse/ HCP and wider team to assess, plan, organise and deliver clinical care in line with service and patient needs, as agreed with supervising clinicians.


The HCA should have a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings.

The HCA should be able to work independently under the leadership of registered nurses,working within the sphere of nursing and care and within all aspects of the nursing process.


HCAs are equipped with the knowledge, understanding, skills, attitudes, and behaviours relevant to employment as an HCA and will work to a nationally recognised Nurse and Midwifery (NMC) code of conduct and must meet the NMC standards of proficiency.


The post Holder will be based at Hillside Primary Care Centre but will be required to travel to different sites often and work in a variety of settings including community centres, clinics, patients home or own home.
