    Location: Brierley Hill Health & Social Care

    Falls Advisor

    Salary: £26,421.00 to £28,770.00 per annum subject to pro rata (30 hours per week)

    To contribute to the provision of the integrated falls pathway as a member of the community falls service. You will work with people who have fallen or have been assessed as being at high risk of falling. You will be giving information and advice to minimise the risk of falls and also carrying out a multi factorial falls risk assessment with the person via phone / in the person's home or community setting and facilitate relevant interventions to reduce falls risk and promote independence and wellbeing.

    At all times to work in a person centred, strength based way to maximise and embrace each individuals own strengths and support network.

    The Council is committed to safeguarding and promoting the welfare of vulnerable groups and expects all staff and volunteers to share this commitment.

    Adult enhanced DBS check
