    Description
    Responsible for monitoring & reporting mechanisms to evidence national & local standards are met and support effectiveness of the service
    To ensure discharge planning and pathways are in place in the Trust and are systematically followed.
    Project Management qualification
    Good, working knowledge of NHS systems, process, data definitions and standards.
    Experience of working as part of a multi-agency team to deliver performance targets.
    Experience of delivering to performance management targets and tight deadlines.
    Knowledge of HR policies and protocols
