About ACCG
We are delighted to offer an exciting opportunity for an experienced Community

Connector to join the ACCG a registered charity providing health and social care services in Manchester.

We are looking for an exceptional individual who is enthusiastic, motivated and experienced to be part our team.


You will connect and engage with people who have social, emotional or practical needs, to improve their health, wellbeing providing support and guidance within the community.

You will support individuals to build social networks, particularly within the area of

Trafford, enabling them to participate in activities in the ACCG and community to improve their health and reducing social isolation.

You will identify gaps in service provision, community activities and groups and feed
- back to ACCG


About the role
Identify and connect with people who may experience being socially isolated.

Help people to connect and access ACCG services.


Establish and maintain Social Support Network for people with specific aim to help them to reduce social isolation, encouraging interventions such as attending group activities, community events and workshops.


Arrange meetings with service users in their home, communities or ACCG Day Centre as required to assess their needs, interests, considering a holistic approach to address all health and wellbeing needs.


About you
Level 3 in Health and Social Care or equivalent qualification is essential

Good verbal communication / language skills

Have an understanding of mild to moderate mental health problems

Be able to work independently and part of team

Ability to represent service users and act as their Advocate as required.

Ability to maintain accurate records within an electronic database

Good IT skills including MS Word, MS Outlook and familiarity with web browsing.

Experience of working with statutory and voluntary organisations.
