    Job summary

    The post holder will be responsible for the development of the highest standards of Clinical Tissue Viability/Leg Ulcer nursing practice within West Essex. The role will be to empower all grades of clinical staff to competently assess and manage all types of wounds of varying complexities. The post holder will assist with the daily running of the Tissue Viability/Leg Ulcer Service in the delivery of a high quality clinical service within Private Nursing Homes and NHS Provider Units. The post holder will work in collaboration with the Integrated Care System to produce wound care pathways and a wound product formulary. The post holder will provide professional advice, support and education to the multi professional team, patients and carers based on NICE Guidelines, the East of England and Midlands Pressure Ulcer Ambition and best practice recommendations in relation to wound management and Leg Ulcer strategies ."

    Main duties of the job

  • To act as a resource providing specialist and expert advice/support for patients, carers and other healthcare professionals in the treatment and management of complex wounds.
  • To maintain appropriate records and systems electronic patient record
  • To act as a clinical resource providing informed advice and support for patients, carers and other healthcare professionals in the management of complex wounds.
  • To participate in the maintenance of local and national service initiatives to provide the highest standards of care in line with accepted best clinical practice and the aims and objectives of the Trust and the wider National Health Service.
  • Participate in Nurse Led Leg ulcer clinics and undertake home visits as required.
  • Plans, provides and evaluates the learning environment for non-registered, pre-registered, newly qualified registered nurses
  • Participate and contribute to developing systems and structures that support learning and development for service users people with complex wounds
  • Participate in the Trust's clinical development education and training programmes.
  • To support the National Tissue Viability Forum with the cascade of relevant information at Trust level.
  • To contribute to the facilitation of the development of Tissue Viability/Leg Ulcer within general practice, organising and planning training agendas, supporting good practice.

    Job responsibilities

  • Undertake comprehensive assessments of the health and psych-social care needs of patients complex wounds.
  • Plan, implement, monitor and review plan of care for service users with complex wounds
  • Recognise the un-well individual and respond appropriately.
  • Undertake dressing management reviews and provision of prescriptions as required.
  • Develop individual, evidence based holistic care plans with the patient, carers, relatives and health care professionals and social care.
  • Work collaboratively with colleagues and use data bases to actively seek out patients who will benefit from on-going Tissue Viability/Leg Ulcer advice and support.
  • Proactively monitor complex patients on the Tissue Viability/Leg Ulcer
  • Prioritise referrals for assessment and management according to their health status and need.
  • Provide and receive clinical supervision in order to continuously improve the quality of care to patients.
  • Demonstrate clinical judgement, acting autonomously in a variety of contexts, within the community setting, ensuring that patients are referred for medical assessment and diagnostic procedures appropriately.
  • Facilitates or participates in multi-disciplinary meetings as required.
  • Collaborate with service providers, patients and carers to develop seamless and holistic pathways.
  • Work with others to identify service users at risk and work towards admission avoidance.
  • Communicate with individuals, groups and communities to improve the health of the population

  • Interpret and discuss assessment outcomes with patients, carers and other health and social care professionals.
  • To attend and contribute to the Skin Matters and represent the Tissue Viability/Leg Ulcer team at appropriate meetings locally, regionally and nationally.Establish effective working relationships with patients, their families and carers. This will include promoting individual rights and recognising and respecting their contributions to care planning and delivery. Using high level communication skills, communicate highly sensitive clinical and complex issues with patients and carers, in particular the skills needed for cognitive assessment and mental health status or when palliative treatment is the final option. Provide telephone support, assessment and advice to people with complex wounds on their management in accordance with national and local policies.

    EDUCATION/QUALIFICATIONS

    Essential

  • Completed study at Diploma/Degree level
  • Evidence of other post registration education and training relevant to Wound Care
  • Non-Medical prescribing or willingness to undertake
  • Desirable

  • Relevant Tissue Viability/Leg ulcer Qualification
  • Managerial Experience

    Essential

  • Experience of working as part of a multi-disciplinary Primary Health Care Team.
  • Demonstrable experience of providing care in a community setting working with patients and carers
