    Job summary

    We are piloting and developing a new innovative pre-diagnostic hub clinical nurse specialist service at Colchester Hospital and this is your chance to be part of its development and make a difference to our patients on cancer pathways.

    We are looking for a Cancer Support Worker, who is interested in working in oncology, to join the team and develop their skills.

    The hub is being created to coordinate faster diagnosis and the safe tracking of patients with suspected or a confirmed cancer diagnosis.

    We are testing a new way of working by merging the pre-diagnosis service with non-site specific and developing pathway management for patients included on the GRAIL Trial, Cancer of Unknown Primary (CUP) and Malignancy or Metastatic disease of Unknown Origin (MUO).

    The pilot's goal is to improve referral and diagnostic pathways. The hub will improve patient experience, access to services and waiting times on diagnostic pathways. The aim is to achieve the national objective of earlier diagnosis and support patients safely through this part of the pathway. Patients will be identified through AOS, Patient Tracking Lists (PTL) and liaison with the cancer performance and clinical teams.

    This post is fixed term until 31st March 2025. Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Please ensure you have discussed this with your line manager.

    Main duties of the job

  • Identify barriers to accessing care for patients who are referred on a target cancer pathway, and works collaboratively with primary care, cancer booking hub, MDT co-ordinators and clinical teams to reduce the impact of this and find solutions which will benefit the patient.
  • To ensure that person centred care is prioritised, and patients' and their family's needs are listened to during the initial stages of the cancer pathway
  • To support clinical teams by facilitating patient access to various diagnostic tests and target appointments and support co-ordination of the pathway in accordance with agreed guidance
  • Help people with suspected or undiagnosed cancer by providing a listening ear service, making onwards referrals and/ or signposting to local supportive services
  • Provide supportive conversations to those who have suspected cancer, offering personalised emotional and practical support and advice by assessing needs, providing relevant information and helping to navigate the patient through the early stages of the cancer pathway, escalating concerns of a clinical nature.
  • Create good working relationships by liaison with Clinical Specialist Nurses, Allied health professionals, primary care colleagues, cancer hub staff and MDT co-ordinators, cancer wellbeing and information centres, clinicians and other MDT members.
  • Record confidential information by completing databases, templates and process flows to ensure project progress can be monitored and impact assessed

    Person Specification

    Qualifications

    Essential

  • Ability to study to level 3 or equivalent ('A' level, advanced diploma, AS level, level 3 NVQ, level 3 BTEC, national award etc.)
  • GCSE or 'O' level Maths & English grade C or above or Level 2 Adult Literacy & Numeracy qualification or equivalent
  • Desirable

  • NVQ 4
  • Foundation Degree/Post Graduate Diploma
  • Communication training course
  • Experience

    Essential

  • Currently employed in a relevant health care or social care background, Health Care Assistant or equivalent, Social care, OT or charity/ 3rd sector services
  • Ability to perform specific job role competencies
  • Ability to work to clear protocols/guidelines within role boundaries
  • Ability to problem solve
  • Demonstrated ability to prioritise and work under pressure
  • Basic computer skills and experience
  • Desirable

  • Previous experience within the NHS and or Primary care
  • Experience being directly involved in patient care delivery either remotely or in person
  • Excel, Microsoft and database skills and experience
