Please refer to the attached job description and person specification for full details of responsibilities. Deliver high-quality, evidence-based care in Stroke Rehabilitation. Lead the development of clinical practices within the Integrated Community System (ICS).Act as a key liaison among the Older People's and Community Directorate (OPAC), CPFT, and external partners to innovate and improve stroke rehabilitation pathways and services.
Support service delivery with flexibility and adaptability. Must be a qualified Advanced Clinical Practitioner with relevant experience in stroke rehabilitation.Ability to travel independently across work localities; a valid driving license and access to a car for work purposes are essential.
Demonstrated ability to work effectively in a multidisciplinary team.Excellent communication and leadership skills.
