- Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community.

- Assist the Rehabilitation and recovery service and the ESTR team to assess, plan, implement and evaluate mutually agreed goals and recovery plans.

- To work with individuals to facilitate access to housing, benefits, and promote engagement in a range of community based activities, working across boundaries of care and organisations.
- To work with service users on a regular and consistent basis to achieve their identified goals, and access appropriate services and resources as required.
- To positively promote social inclusion and active citizenship and developing social capital for service users
- To support the person to reclaim or develop valued social roles which may not have been available to them whilst they were emotionally or mentally unwell.
- To positively promote the values and principles of recovery, focussing on strengths of the individual and inspiring hope.
- To provide support to individuals with daily living, which may involve practical help, together with emotional/psychological support and assistance with any difficulties as they may arise.
- To develop a rapport with the service user based on trust and respect, but within appropriate and professional transparent boundaries
- To help service users gain access to resources, including housing, benefits, and welfare rights.
- To provide information and promote mental and physical health and wellbeing.
- To help identify the Early warning signs of relapsing mental health and alert the appropriate staff involved in the care
- To attend ESTR clinical planning meetings
- To report regularly to Senior practitioner and to participate in managerial and clinical supervision
- To attend and participate in CPA review meetings as appropriate
- To develop a sound knowledge of resources available locally, and develop good relationships which will support service users in their recovery, education providers, housing, benefits, employers, leisure, voluntary agencies.
- To support the service user to represent and express their views utilizing effective modes of communication, including written reports.
- To provide the personal details with the service user in order to develop the person centred care plan.
- An ability to demonstrate self motivation.
- An ability to assess and evaluate individual needs to enable the service users to make informed decisions
- An ability to analyse complex information concerning the legal aspects relating to housing and benefits
- Certificate in Community Mental Health Care / NVQ Level 3 in care or agreed equivalent Communicate to undertake CPD Good literacy and numerical skills
- An awareness of issues affecting mental health service users Knowledge of approaches to working with mental health service users A knowledge of housing and welfare benefits, housing issues and related employment and educational issues Experience working within a team Experience of monitoring and recording information

Main duties of the job
