    Job Title: Highly Specialist Community Physiotherapist at Dorset HealthCare


    We are looking for a Highly Specialist Community Physiotherapist to join our Integrated Community Rehabilitation Teams covering the West Dorset Cluster. Our teams work with adult patients in their homes, Community Hospital Wards, and Care homes to promote independence and improve their quality of life by supporting them to return and remain at home.

    The role of Highly Specialist Community Physiotherapist involves providing highly specialist assessment, advice, and interventions to prevent unnecessary hospital admissions, support timely discharges from the Hospital, and offer Rehabilitation programmes for patients in a Community setting. You will be responsible for holistic clinical assessments of patients, making decisions about their treatment, goals, and discharge plans. Additionally, you will support junior staff with complex case management and clinical supervision when needed.

    We offer opportunities for further training and development along with the chance to take part in Service development projects.

    Employment in this role requires an Enhanced DBS check, which the Trust covers the cost of. Applicants subscribed to the DBS update service can present a valid certificate instead of requiring a new check. Moreover, a relocation package of up to £10,000 is available for those interested in moving to Dorset.


    Main Duties of the Job:

    • Undertake highly specialist autonomous assessments, interventions, and management of patients independently and as part of a multidisciplinary team.
    • Provide highly specialist advice on patient management and care to others.
    • Respond to patients in crisis in their homes to prevent unnecessary hospital admissions.
    • Offer follow-up and monitoring through clinical assessment and interventions to help people return and stay at home.
    • Devise specialist care programs for delivery by other professionals and ensure their proper implementation.
    • Provide high-quality, person-centered care that prioritizes safety, privacy, and dignity.
    • Deliver specialist rehabilitation programs to patients in their homes, community hospitals or care homes.

    Knowledge, Skills, and Training:


    Essential

    • Registered practitioner to degree/diploma level supplemented by Masters level study or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty.
    • Learning and Assessing in Practice Qualification or equivalent practice assessor training.
    • Evidence of recent professional development in an up-to-date portfolio.
    • Membership of the relevant Professional Body.
    • Evidence of Leadership training and application in practice or willingness to complete.
    • Broad clinical and multi-agency knowledge.

    Job Specific Experience:


    Essential

    • Proven experience at Registered Practitioner Band 6 level in the community.
    • Demonstrated specialist expertise and knowledge supported by theory acquired through CPD related to clinical assessment, treatment, and evaluation.
    • Active involvement in the supervision and development of multidisciplinary team members.
    • Participation in developing care programs, protocols, and audits.

    Personal Qualities/Attributes:


    Essential

    • Effective communication skills at all organizational levels and with various stakeholders, showcasing negotiation and persuasive abilities.
    • Capability to analyze situations and navigate conflicting scenarios, selecting appropriate actions when faced with multiple options requiring judgment.
    • Experience in planning and organizing complex activities, such as coordinating with other agencies and handling unforeseen circumstances.
    • Evaluation skills to improve care quality and service standards.
    • Initiative to make decisions within the defined scope of authority.

    Essential

    • Proficient in written and spoken English to effectively carry out all role responsibilities.
    • Ability to travel using a personal vehicle for Trust business.
