We are searching for a Health and Social Care Assessor to operate in the Evesham and Pershore areas, including surrounding villages.


Main tasks of job:


  • Complete reviews of existing care plans and risk assessments
  • Liaise with health and social care professionals
  • Liaise with the management team and care staff on service users' changing needs
  • Work in a personcentred manner to promote service user independence
  • Any other duties that are required in line with business needs

Minimum Requirements:

Experience with Microsoft Office

Any work experience in Health and Social Care Assessment

Excellent communication skills


Desirables:

1 year's work experience in Health and Social Care Assessment

NVQ Level 3 Health and Social Care


Job Types:
Contract, Full-time, Part-time


Salary:
£11.50-£12.50 per hour
