Description

The post holder will be a member of a multi-disciplinary Adult Social Care team, working under the direction of the Office Manager.


The post holder will undertake a range of fieldwork support duties in order to enable and assist people in the community to access appropriate services and maximise their own, or their family's independence.


This is what we need you to do...

  • You will be expected to undertake assessments, in line with the Care Act 2014 and the national eligibility criteria.
  • Provide service users with advice, support and information on sources of help within the community, utilising the services of the agencies as appropriate.
  • Liaise with Health and Social Care professionals, and with statutory and voluntary agencies to produce appropriate support plans to meet the needs of service users.
  • Review the needs of service users as necessary or to assist in maximising promoting independence.
  • To communicate effectively within the team and engage with all appropriate stakeholders across the organisational boundaries to enhance job performance.
  • To produce succinct, wellstructured records and reports, clearly recording and reporting analysis and judgement. In addition to maintain appropriate and timely records on work undertaken in adherence with Data Protection.

Special Conditions

  • This post involves travel throughout Worcestershire.
  • This post may occasionally involve work out of normal office hours.

Experience Required

  • Experience of working with vulnerable adults and in responding to their needs in a social work/care setting.
  • Experience of managing a caseload, ideally including working with people experiencing a range of physical disabilities, medical, mental health difficulties and learning difficulties.
  • Experience of undertaking assessments of needs

Knowledge, Skills and Understanding

  • Can demonstrate the ability to risk asses, plan, evaluate and arrange services.
  • Can demonstrate the ability to work under pressure to problem solve through prioritisation, negotiation and decision making skills.
  • Has evidence of good interpersonal skills, good oral/written communication and numeracy skills.
  • Has evidence of good keyboard skill and the ability to use various IT packages.
  • Understands current legislation framework in reference to the Care Act, e.g., eligibility/MCA/Safeguarding, relevant to the demands of the post required.

Education & Qualifications

  • Essential_
  • Training related to Health and Social Care assessments.
  • Holds relevant qualification equivalent to NVQ Level 3
