    Employer West London NHS Trust Location Southall Salary £49,178 to £55,492 per annum inclusive of HCAS Closing date 28 May 2024

    The post holder will work closely with fellow Clozapine nurses, Phlebotomists and Community Matrons across the 3 boroughs to ensure high quality service delivery and cross cover across the three boroughs.

    The role will also involve supporting Clozapine staff to provide a wide range of physical health monitoring and treatment and to participate in development of the Clozapine service across the three boroughs.

    The role will involve coordinating Clozapine clinics and liaising closely with patients, carers, inpatient units, CPNs, community teams, GPs, Consultant Psychiatrists and others to ensure the wellbeing of patients and to promote the general understanding of clozapine clinic process, medication and monitoring issues.

    Performance monitoring and management of the staff team

    To deliver a service within the agreed financial envelope ensuring maximum value in terms of clinical and cost effectiveness.

    Ensure that a culture of effective risk management is present in the team

    The Trust leads the NW London Children and Adolescent Mental Health provider collaborative.
    Registered Nurse with current registration [mental health], appropriate to the job role
    Evidence of continuous professional development
    Phlebotomy training

    Experience of working alongside people with complex health problems in the designated care group
    Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
    Experience of conducting clinical assessments including risk assessments
    Experience of working within the Complex case management process or other health, care process systems for shared care and enhanced care with a Mental health client group
    Previous experience of working in the community
    previous experience of contributing to the implementation of new service interventions and processes.
    Experience of working within a Mental Health setting
    Experience of giving professional development and appraisal.

    Knowledge and understanding of the principles of recovery and trauma informed care their application to risk management, positive risk taking and strengths orientated approaches to care delivery.

    Knowledge of the principles of co production in mental health care
    Knowledge of primary care networks
    Understanding of CQC regulations

    Knowledge about clozapine and other medications, practices and process around the safe administration and monitoring of clozapine and other medication.

    Knowledge about health and safety at work procedures
    Knowledge about Safeguarding adults and Children procedures
    Knowledge of the Mental health act and governance structures
    The ability to promote positive inclusion and participation.
    Physical health assessment skills. Able to use physical health equipment (Blood pressure machine, BM machine, etc).

    Able to document physical health results and act on physical health concerns with awareness of the parameters of physical health (blood pressure, Blood sugar, Lipids, BMI etc) and escalation actions.

    Mental health assessment skills - able to complete a mental state examination and assess mental health and support needs of patients and refer to appropriate services.

    Organisational skills - able to organise clinics, involving appointments to administer clozapine medication to patients, Able to prioritise and make arrangements for when patients have missed appointments.

    Phlebotomy skills able to take bloods from patients and send for analysis.
    Able to use clozapine clinic equipment effectively, including machinery used to analyse blood results(POCHI), maintain maintenance of this equipment and report concerns
