Main Duties of the Job:
About the Role: An exciting opportunity awaits for a passionate nurse to join the Locala Diabetes Service. We are ideally seeking a Band 6 Diabetes Specialist nurse to make a real impact on individuals' health and well-being. However, this position is also open as a Band 5/6 development role.
The role calls for a motivated individual interested in personal and professional development to provide top-quality diabetes care in the community. You must demonstrate awareness of national and local issues and a dedication to collaborative work with other providers.
Working closely with GPs, mainstream nursing, social, and secondary care services, you will adopt a patient-centric, coordinated approach to case management in primary care. Your involvement will influence health and social services efforts to enhance patient choice, quality of life, and self-management, following a community care team approach.
