    ABL Health andNottinghamshire County Council are partnering to deliver and IntegratedWellbeing Service across Nottinghamshire. This transformative servicewill play a central role in system change, bringingpreviously separate lifestylebehaviour change functionstogether into one service. It seeks to work collaboratively with communities tosupport the transformation of services for the local population. The servicewill provide integrated behaviour support for smoking cessation, weight management, alcoholreduction, physical activity and falls prevention, all embedded insustainable behaviour change and positive wellbeing.

    Job responsibilities

    Role Purpose:

    You will be thePartnerships & EngagementOfficer for a specified geographical area for the YourHealth Your Way Integrated Wellbeing Service for Nottinghamshire County.

    You will beexpected to engage at neighbourhood and place level to increase referrals intothe IWS and build long- lastingrelationships with local partners. This will involve being responsible fordeveloping and maintaining strong,positive and long-lasting relationships with a wide range of stakeholdergroups, including but not limited to local authority, primary and secondarycare, leisure services and the community andvoluntary sector and communityservices.

    You will be a motivated,passionate, organised and proactive communicator, with experience working in a community setting. ABL take a strengthsfocused asset-based approach to community delivery, looking to innovate and partner, not replicateexisting delivery. The role will require dynamic working which embraces the visionand values we wish to establish for the IWS supporting people to achieve healthy lives.

    No two dayswill be the same in this varied and flexible role. If you are as comfortablespending a day with the team as youare working independently and confidently with new people and are highlyorganised this could be thejob for you.

    This is a demanding job, but youwill not be on your own. ABLs highly experienced and friendly multi- disciplinary team of Health and SocialCare professionals, Business Intelligence, Service Development, Governance, Finance and BusinessDevelopment are thereto support. Additionally, existing experienced delivery teams will be available tosupport you to drive the Service forward.

    Duties and Responsibilities

    This post willwork under the line management of ABLs Head of Strategic Partnerships. The role will be varied,challenging and rewardingas you support and developthe service throughimplementation, delivery and ongoing innovation and service development.

    Key responsibilities will be:

    Implementing the ABL Community Engagement Strategy to promoteIWS

    Working with ABLs communicationteam to help raise the profile of IWS.

    Engaging local services and partners to ensure localawareness and explore partnership opportunities and co-productionactivities.

    Working closely with operational colleagues to managethe referral rates and develop partnershipswith all potential referrers in your locality.

    Organising/attending GP practice/PCN meetings to promoteIWS and sustainreferral pathways.

    Attending local health forums/groups/meetings on a regular basis whererequired to represent ABL, promoteIWS and maintain local partnerships (this will involve some publicspeaking/presenting to small/medium groups of people)

    Working collaboratively with all members of the Nottinghamshire IWSteam and undertaking work in otherareas when necessary, includingoccasional delivery support.

    Supporting the monitoring and evaluation of the service and usingformal and informal feedback to constructivelyshape and develop the service.

    Update the partnership database with all contacts including requiredfollow up contact and strategic developmentsrequired

    Attending all relevant team meetings including district team meetingsand community engagement meetingsproviding engagement updated and contributing to quarterly reports as directed

    Reporting against relevant targets, providing regular updates asrequested on progress of project.

    Supporting with communications strategies ( public open events,training and awareness raising sessions,communications materials, social media etc.) to promote the service and capturereferrals (occasional weekend work may be required to support with events etc.)

    Innovative and open minded to new ideas andpiloting of different sessions

    Supporting all co-production activities with local residents andstakeholders to support ongoing developmentof the service

    Ability to coversessions as and when requireddue to sickness or annual leave.

    Essential

  • Degree and/or equivalent experience community healthcare, psychology, behaviour change

  • At least 3 years experience in a healthcare setting setting.
  • Experience of engaging with a wide range of strategic and operational partners
  • Experience of working in a health care environment
  • Demonstrable experience of speaking to large groups
  • Experience of managing a team in a target driven setting.
