- Supporting young people from point of entry into the service and through their journey.
- Providing screening, assessment, psychosocial interventions, integrated care planning, post treatment support, including transfer of care.
- Identifying and responding effectively to potential safeguarding issues.
- Reducing drug and alcohol related harm to young people and the wider community.
- Promoting carer, young persons, and community involvement.
- Providing advocacy for access to partnership services.
- Working with young people to support positive, holistic outcomes in relation to their health and wellbeing, enabling them to lead safe, healthy, and purposeful lives: reducing risk and increasing resilience.
- Experience of working with young people and knowledge of the issues they face.
- Knowledge of safeguarding policies in relation to children and young people and the Fraser Competence framework
- An understanding of drug and alcohol issues with experience of working within a related field.
- Knowledge of working with evidence-based practice around young people's drug & alcohol services and methods, including relevant best practice guidance.
- An accomplished communicator, both verbal and written with a high degree of personal IT competency.
- Be able to accurately update and maintain records and to work to deadlines for the submission of information, ., reports.
- Have excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
- Drivers licence and access to a vehicle is essential due to the role being countywide.

Location: Derbyshire, countywide
Salary: £27,449,32 - £31,529,41 per annum
Hours: 37.5 per week
Responsibilities
About the job:
You will play a crucial role at Change Grow Live, guiding young individuals through a range of interventions tailored to their needs. You will equip them with essential skills and knowledge to navigate decisions regarding drugs and alcohol. Your role involves fostering resilience in young people, encouraging safer, happier, and healthier lifestyles. Being able to travel across Derbyshire is important, so having access to a car and possessing your own driving license is necessary.
