Monday - Friday 08:30 - 16:30 + back up

Salary:

£12.90 per hour

We work alongside the NHS to provide excellent hospital to community discharge as well as providing private care, ensuring that the normal day to day activities of daily living are achievable despite diagnosis and prognosis.


As one of our Care Coordinators you will:


  • Visit Clients and make reviews/audits of the written Care Plans, examine the written records of medication administration, finance and daily care records kept by Staff.
  • Use our rostering system to rota calls and cover shifts with suitable staff.
  • Ensure that each service user receives care appropriate to their individual need.
  • Liaise with health care professionals and relatives about the provision of care to service users.
  • Assist with the on call duty, working with the Care Coordinator team to ensure 24/7 cover, including weekends and bank holidays.
  • Carry out spot checks, supervision and appraisal reviews of the staff's performance.
  • Carry out the monitoring of the service in the field to ensure that it meets the quality targets of the Company, contractual requirements of the commissioners and the NHS.
  • Excellent record keeping on electronic systems.
  • Have experience in care provision at a level that reflects the responsibilities of the post applied for.
  • Have used rota software previously and covered shifts as part of their role.
  • Have good literacy and numeracy skills.
  • Have good presentation skills.
  • Be computer literate and able to use various systems and databases.
  • Have an understanding of legislation concerned with care provision.
  • Have administrative experience.
  • Be able to provide care to all clients across the service as necessary.
  • Be able to reallocate work to staff to cover sickness and holidays.
  • Have a full driving license (able to have business insurance).
