    We have an exciting opportunity to recruit a band 5 registered mental health nurse to work as part of our older peoples community mental health team.

    The service is based on Summer Lane, Wombwell, Barnsley and operates Monday to Friday, 9am to 5pm.

    We are a specialist secondary care service providing community mental health care to service users with varied and complex mental health needs alongside co-existing physical health needs.

    You will work as part of the MDT and also independently under guidance of senior practitioners to provide high quality, evidenced based care to service users in their own homes, across the Barnsley locality.

    Registered with the NMC as a nurse, you will have experience of working with severe and enduring mental health conditions, excellent clinical skills, effective communication and working collaboratively with service users, their families and other services.

    Working in a recovery focused approach, you will participate in the assessment, planning and implementation of evidence-based care, medication administration and physical health monitoring.

    As a NMC registered Mental Health Nurse you will:
    *Recognise and work within your competence, be accountable and responsible for your actions, in line with the NMC.
    *Work as part of the MDT but also independently, under the guidance of senior practitioners within the team.
    *Comprehensively and holistically assess the needs of service users with severe and enduring mental health conditions, then working collaboratively you will plan, implement and review evidence-based care.
    *Identify and positively support and manage risk with service users and their families. This will include recognising and reporting safeguarding.
    *Undertake physical health monitoring which includes blood pressure, venepuncture, ECGs.
    *Support doctors and senior practitioners to monitor prescribed medication.
    *Liaise with families and carers, MDT, other secondary care services, primary care and third sector organisations.
    *Supervise and support junior members of staff and student learners.
    *Maintain all mandatory training, identify own professional development needs and access training as required.

    The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload.

    To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating skilled interventions to address identified needs.

    To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered e.g.

    To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.

    To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.

    To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met.

    Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
    To be responsible for the safe custody, transportation and administration of medicines including I.M. preparations.(Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity. To identify any areas of concern to a senior member of staff.
    To participate in the Trusts values based appraisal system, understanding appropriate training and participating in personal development opportunities.

    Experience of working holistically with adults with an emphasis on coexisting mental and physical health needs that impacts on wellbeing.

    Registered Mental Health Nurse / Social Worker / Occupational Therapist


    TRAINING
    Assessment and care planning skills and risk assessment and risk management.

    Relevant post basic qualification and training.

    Mental Health Act, Health & Safety, Care Programme Approach, Care Act & Safeguarding.

    Knowledge of working collaboratively with people and their carers to ensure their involvement in the assessment and care planning process.

    Shows awareness of cultural diversity and is able to incorporate this into care planning and the delivery of care.

    Ability to work on own initiative, with reference to policies and procedures.

    A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
