    Full Time - 37.Patient flow is critical to patient safety, health, and patient satisfaction. With improved patient flow more positive health outcomes can be expected. Any admission to an in-patient health service in mental health should be purposeful, therapeutic and safe.

    The Patient Flow Team supports the monitoring of hospital admissions, discharges and clinical ready for discharge (CRFD) but delayed patients as well as movement across inpatient services.

    This provides accurate, up to date information to enable people to have access to a bed when required without compromising patient safety, care and treatment.

    The Patient Flow Team support acute inpatient care for both working age adults and older people.

    This service covers the four geographical areas of Barnsley, Calderdale, Kirklees, and Wakefield and operates as a trust wide bed base.

    All employees of the Trust are strongly encouraged to be fully vaccinated against COVID-19 to protect patients.
    Ensuring that inpatient services are fully joined up across the organisation, with patient flow operating as one team covering working age adult acute, older peoples acute and psychiatric intensive care unit (PICU) wards
    Working collaboratively to achieve safe and timely admissions considering, demand and capacity, mixed sex accommodation, infection control, etc
    Working closely with internal and external teams to ensure discharge planning is proactive and commenced on the point of admission, enabling any barriers to discharge to be identified and acted upon in a timely manner with an estimated discharge date (EDD) being agreed upon as soon as possible (as part of the 72 hour review)

    The Patient flow and bed management service will have overall management of the acute wards demand and capacity on a day to day basis and senior level responsibility for patient flow.

    The post holder will be responsible for the management of all inpatient beds and to ensure the safe admissions for Adult Acute patients across the Trust.

    The postholder will be required to interpret national and local policies and guidance and will provide advice on how they should be interpreted by staff in the patient flow teams ensuring the highest professional standards are maintained at all times.

    The postholder will ensure the highest standards of patient flow are provided and that the principles of clinical governance are implemented, utilising their authority and judgement to solve immediate operational problems.

    The post holder will take the lead in ensuring that analysis on bed usage, demand and capacity for admissions is understood, that data is collated and that trends are mapped and communicated to General Manager.

    The post holder will work as part of a collective senior leadership team working alongside the general manager & modern matrons to achieve patient safety and regulatory assurance, whilst leading, innovating and managing the development of improved governance, performance and quality improvement both within each BDU and where appropriate Trustwide.

    Lead, motivate and develop staff in all aspects of effective admission and discharge planning.

    Focus on ensuring that they develop excellent communication skills with all inpatient wards, Team Leaders, Modern Matrons, Medical staff and the General Manager for Acute Care.

    Will ensure that there is up to date, visible and accessible information available at all times on occupancy levels, admissions and discharges and that all digital solutions to patient flow are developed and utilised to support this.

    Ensure that Operational Pressure Escalation levels (OPEL) are embedded and contribute to the implementation of safer staffing and internal professional standards at ward level and ensure that escalation processes are part of the operational meeting structures.

    ~ Have responsibility for ensuring that all patients have a registered GP within the Trust footprint and have been gate-kept for an admission as per Trust policy.

    Over reliance on AI-generated content in application forms is strongly discouraged and we will conduct a thorough screening process before selecting candidates to progress to the next stage.

    Relevant graduate qualification or equivalent.
    Relevant post-graduate qualification

    Training
    Relevant post qualification training / learning in professional field.
    Willingness to maintain mandatory training requirements

    Safeguarding manager and/or investigators training

    Has advanced knowledge and proven ability in current mental health practice and national policy and guidance

    A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).

    A Current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability)
    Ability to demonstrate a high level of skill in supporting/mentoring/supervising staff and students/trainees.
    Ability to demonstrate a high level of skill in assessing and managing risk in a variety of settings
