About Jane Ford:

I regard myself as a responsible, conscientious NMC Registered Nurse with experience and knowledge in the Acute, Primary Healthcare and Educational sectors with more than 16 years of clinical and case management experience. My career to date has incorporated many roles and responsibilities spanning from RGN, District Nurse, Practice Nurse, Health Advisor/Vaccinator to a School Nurse & Wellbeing Advisor. My current role encompasses a range of case management services, including health assessment, diagnosis and expert medical advice.

School Nurse/Wellbeing Advisor – February 2022 – To date

In my most recent role as a School Nurse (Band 7), I support pupils and staff members with disabilities, chronic and acute health conditions, formulating a range of case management services, including health assessment, diagnosis and expert medical advice. Health care plans and liaising with the pastoral team, community primary health care team and advisory services on a regular basis. I advocate Mental health and psychological services and signpost as required. I complete drug and alcohol assessment’s and recommend sickness absence procedures and fit work for work guidelines. I work in collaboration with teachers and the Pastoral Deputy Head to complete risk assessments for pupils and staff with chronic and acute conditions. I liaise directly with Primary Care Teams and former educational settings to submit referrals as required. I liaise with parents and staff in accordance with School policies on day-to-day issues regarding the medical needs.

As a Health Advisor (Band 7), I directly support patients in gaining access to the medical professionals and specialists they require. Conducting clinical assessments and case management reviews for the safety and wellbeing of the service user. With excellent communication skills, I would gain trust, consent, and confidence of each patient, ensuring the best possible clinical outcomes while adhering to up-to-date policies and procedures.

Practice Nurse – March 2021 – June 2021

My role as a Practice Nurse, medical assessment and case management reviews of people of all ages, including Audiometry, Spirometry, Vision Tests, Skin Inspections & HAVS. Diabetic and Asthma clinics, phlebotomy, immunisations. Working autonomously with a Multidisciplinary Team within General Practice as part of the Primary Healthcare Team. This included completing patient consultations, physical examinations, and investigatory procedures. I provide comprehensive wound care, administrator medication, diabetic assessment including insulin management, emergency first aid and treatment.

During my role as a Mentoring, Education and Training Facilitator, I was responsible for supporting and encouraging pupils in their schoolwork through a holistic approach. My role as a Community Nurse included the assessment and implementing of patient care to improve the health and wellbeing of communities and educate them about illness and disease prevention. Seamless nursing care to housebound patients including wound care, catheter care, diabetes management, injectables and End of life care. To be responsible for admitting and discharging patients from the caseload and documenting through EMIS accordingly.
