We are recruiting 3 nurses to start on 5th November 2024.
LOCATION: Various locations across Leeds (allocated GP Practices to be confirmed).
Own transport is highly recommended for this post.
This is a training post. However, you will be employed as a PracticeNurse by Trust Primary Care Ltd (TPC) on a fixed term 12 month contract with anannual salary of £27,000 (includes access to the NHS pension scheme).
You will be working in a GP practice that is approved as a centre of excellence in education and training. You may also have the opportunity to work across other sites and/or elsewhere in their Primary Care Network (PCN), to broaden your understanding of general practice and how different surgeries can operate.
In the first six months you will complete several short courses to gainpractical skills including: Phlebotomy, Vaccination & Immunisation, Wound Care, CVD, MentalHealth, Asthma & COPD, Diabetes, Learning Disabilities, MotivationalInterviewing, Quality Outcome Framework (QOF) and Contraception & SexualHealth. In the second six months you will complete a long-term condition module. Throughout the 12 months,there will be a monthly peer group learning sessions where the whole group cometogether to learn and share through action learning sets.
You should be:
A final year student nurse / a nurse new to Primary Care.
Able to work hours per week (Monday to Friday)
Able to travel to your allocated GP practice and attend all training courses provided.

Be part of the Practice Health Care team providing high quality andforward thinking nursing care to meet the holistic healthcare needs of thepractice population. To work within NMC Code of Conduct and to agreed practicestandards and protocols.

Job descriptionJob responsibilities
The Practice Nurse will have support from their work place mentor to beaccountable for their own professional actions.
After relevant training, undertakes procedures to include, venepuncture,blood glucose testing, BP checks, urinalysis, peak flow, new patient healthchecks, pregnancy testing, stitch/clip removal.
Recognises and understands the importance of observing, documenting andcommunicating the signs and symptoms of child and adult abuse.
Undertakes regular checking of all resuscitation equipment.
Works within all relevant practice policies and procedural guidelines, infection control, chaperoning, Patient Group Directions (PGDs).
Enables, encourages, and supports individuals, families and groups toaddress issues which affect their health and social well-being.
Represents individuals and families interests when they are not able todo so themselves.
Develops and maintains the health promotion material within thepractice.
Makes referrals to statutory and voluntary agencies as appropriate.
The Practice Nurse has a role in promoting good leadership for allgrades of staff
Contributes to the development, monitoring and evaluation of caredelivery by the Primary Health Care Team.
Promotes effective communication and relationships within the team.
Partakes in clinical supervision for own development.
Actively participates in clinical and practice meetings.
Develops own knowledge and practice to meet objectives/changes inservice.
Contributes to practice policy development as required.
Monitors and uses resources appropriately within the team ,dressings, vaccines, stationery.
Makes effective use of the clinical system by ensuring the consistentand accurate entering of data and use of Read Codes.
Facilitates the use of up to date information technology in order tocollate accurate and timely information as and when required.
Makes recommendations to raise standards of practice and servicedelivery.
Frequent exposure to body fluids, blood and wounds.
Occasional exposure to aggressive behaviour.
Potential requirement to travel between sites.
