Ok a wee bit about me. I have quadriplegic cerebral palsy, with a learning disability. I am registered blind and my disability has a big impact on me physically with a wide range of associated conditions and symptoms.
My personal outcomes
To have a full and good life with lots of fun and laughter. (Most Important)
To continue to have good relationships with my family as they are very important to me.
To have my daily care needs, and associated healthcare appointments, met and to remain safe and as well as I can be at any given point in time
To have a weekly timetable filled with stimulating and enjoyable activities, and to be valued and feel included in what is going on around me
To have a meaningful plan and timeline to transition to my own home when the time and circumstances allow; this will involve co-ordination between me, his family and my support team.
In summar I need someone who will be alongside me to help me get more out of life; to be safe and happy at home and in my community and to live the best life possible with lots of fun and laughter.
The tasks include (but not all of them every time):
Following my support plan.
Supporting me with all activities of usual daily life as agreed in advance with my family.
Follow my schedule of activities after discussion and agreement with my family and the Tartan Sheep Care team.
Taking part in training deemed appropriate by my parents, for this role.
Following the specific protocols to help me manage my health including medication administration, attending appointments and my prescribed physiotherapy. These protocols are recorded in my support plan and are updated as and when required. They form part of this job description and must be followed as appropriate.
Using appropriate infection control measures with adherence to appropriate use of PPE and regular handwashing.
Giving me time to communicate and to help me recognise the context of the situation I am in so that I can join in well.
Supporting me with working out information and to support me with everyday decision making by ensuring you talk to me using short sentences and with reference to things that are familiar to me.
Communicating openly, honestly and frequently with your colleagues, Team Leader and my parents as your employer.
Shopping and preparing meals for me based on my likes and dislikes and to help me meet my nutritional needs.
Helping to keep my home clean and tidy.
Working as a member of a team with other Personal Assistants, the Team Leader and my family.
Any other tasks as requested by the Team Leader or my parents that are appropriate for the role of Personal Assistant.
