    Description

    An exciting opportunity has arisen to join Herefordshire Community Learning Disability Teams as a Band 5- 6 Community Learning Disability Nurse based at Liverpool Court, Commercial Road, Hereford.

    The successful applicant will become part of a small but tenacious and committed team who are passionate in ensuring that individuals with a learning disability receive equitable health care alongside managing an effective caseload and maintaining regular visits for service users and their carers.

    The team are currently involved in some exciting service developments including STOMP, Dementia pathway, transitions pathways to better meet the needs of individuals with learning disabilities.

    We welcome creativity, enthusiasm, and passion in this team with a go for it attitude to best meet patients health needs.

    You will receive regular individual and group supervision, on-going CPD & training, and support within this role to help further develop your skills and competencies in this specialist field.

    We offer flexible working and full equipment provided for home working.

    Due to the rural landscape of the county, being a car driver is essential but there also are good public transport links to the main base.

    Facilitate/teach effective systems to carers to enable the transfer of skills required for the management of service users healthcare needs.

    Facilitate Health Action Plans for each service user on caseload, to enable access to the full range of primary and secondary care services available.

    Maintain accurate and up to date clinical records in line with Trust/NMC standards of practice.

    Ensure the process of referral, assessment, planning, intervention, and evaluation of care is efficiently and effectively delivered with clear quality outcomes.

    Promote the values and principles of person-centred planning approaches, maintaining effective continuity of care.
    Undertake specialist healthcare assessments, ensuring referral evaluation, data analysis and intervention planning is identified to enable service access.

    Implementation, monitoring, and evaluation of therapeutic nursing interventions based on a comprehensive assessment using a recognized nursing model of care.

    A good level of knowledge of current policies and legislations that impact upon people with a learning disability
    A good level of knowledge in a wide range of common health care conditions in people with learning and other disabilities and vulnerabilities including common long term conditions
    Child protection and vulnerable adults procedures
