We are delighted to announce an exciting opportunity to work within the Dragonfly Unit team as a Discharge Facilitator.


The purpose of the role is to maintain momentum in discharge planning throughout the entire process (starting from the time of admission), supporting and working in partnership with patients, their relatives, team colleagues, and other professional agencies to ensure a safe and timely discharge from hospital.

We aim to keep a person's time in hospital to a minimum, constructively challenging decisions, to prevent avoidable delays to discharge so that access to acute care remains available.


This position would act as first point of contact for both internal and external enquiries relating to admission and discharge, facilitating effective communication between those involved in a patient's journey through their discharge and ensuring appropriate community care and follow-up support is arranged for them to remain discharged.


Some duties and responsibilities include:


  • Acting as the key liaison between the hospital and other agencies in preparation for discharges
  • Delivering personcentred support and discharge planning
  • Working within the local area to support patients with their assessments in the community for a safe and effective discharge to the most suitable destination
  • Liaising with external organisations as required promoting partnership working to ensure holistic care and support
  • Arranging, attending, and contributing to meetings
  • Making referrals to relevant community services and other agencies as appropriate
  • Completing daily reports and contributing towards other reporting requirements
  • Ability to work independently on own initiative to meet the needs and preferences of patients
  • Ensuring that patient needs, preferences, and progress are accurately reflected in health record documentation including risk assessments and care plans
  • Assisting patients and their carers to access information, or to signpost as required
  • Constructively challenging a patient's readiness for discharge within a multiDisciplinary environment
  • To have an understanding of Social Care requirements and legislation including the Mental Health Act, Mental Capacity Act, and Care Act
