Description
The Bi Borough Short Breaks service is passionate about identifying disabled children/young people's potential and supporting their independence and participation in their community.
The role entails producing several assessments and care package reviews within timescales.Evidence of analytical writing is crucial, as the ability to process large quantities of information, record the most salient points, and provide a balanced view and evidence to support your opinions whilst producing clear recommendations and "smart" outcomes.
The voice of the disabled child/young person is at the heart of our work.Your role will involve acquiring the voice and views of the child through various communication tools/aids; the child's views must inform your assessments.
Practitioners work across both boroughs (Westminster and RBKC).Assessment and reviews are completed with families at home, and observations occur at schools, play schemes and other community settings.
We work within a hybrid model, expecting to work office base at least three days per week.Job Types:
