An exciting opportunity has arisen for several Band 5 Occupational Therapists to join our rotation covering Neurology, Community, Wheel Chair services and Frailty with East Coast Community Healthcare.
The Integrated Community and Neurology Team - The team supports community patients and patients with long-term neurological conditions across the Great Yarmouth and Waveney area, and consists of specialist nurses, physiotherapists, Occupational Therapists and rehabilitation assistant practitioners. Assessment and intervention is completed in clinic settings as well as in patients homes and care homes. We provide timely assessments and treatment to facilitate and improve patients quality of life and maximize independence. You will take responsibility for a designated caseload of patients, supporting them throughout their journey from initial diagnosis, management and palliative care.
Community Team - The Team support patients in the community with complex care needs who require specialist Occupational therapy input to aid in function, supporting patient to achieve optimal independence at home.
Main duties of the job
The Frailty Prevention Team is a multidisciplinary team consisting of Occupational Therapists, Physiotherapists, Assistant Practitioners, Exercise Professionals and Nurses. The team provides admission avoidance, Frailty Prevention Assessments,
Falls Assessment, Home Hazard and Environment Assessment, Equipment Provision, Strength and Balance training. The Team will endeavor to advise patients, relatives and carers on how best to assist patients to remain well and manage their everyday living. Our aim is to maximise their independence, achieve their goals and to support them to remain safe and well at home.
Wheel Chair Services - You will work with Adults and Children. Our clients have a wide range of conditions affecting mobility and posture. You will gain experiences in completing the following: Triage in line with NHS England guidance, environmental assessments, clinical assessment skills, powered wheelchair driving assessments, gain knowledge of accessories and adjustments that can be made to maximise function, opportunity to gain specialist skills that will transfer into other settings and your future career development.
We provide a structured supervision program, including clinical support and regular in-service training. In return, we expect applicants to show a commitment to Continued Professional Development and evidence based practice.
Location will vary from Gorleston, Lowestoft or Beccles, dependent on rotation.
Part-time applications will also be considered.

CLINICAL:
To beprofessionally and legally responsible and accountable for all aspects of yourown work including the management of patients in your care. To ensure a highstandards of clinical care for the patients under your management and supportothers to do likewise.
Tointerpret and analyse clinical and non-clinical facts to form an accuratediagnosis and prognosis in a wide range of conditions, to recommend the bestcourse of intervention and to develop comprehensive management strategies forthe patient's presenting condition.
Toundertake comprehensive assessment of patients, using investigative andanalytical skills and to formulate individualised management and treatmentplans, using clinical reasoning and utilising a wide range of treatment skillsand options to formulate a specialised programme of care.
To acceptclinical responsibility for a designated caseload of patients and to organisethis effectively and efficiently with regard to clinical priorities and use oftime.
To developthe skills and dexterity, co-ordination and palpatory senses for assessment andmanual treatment of patients. To be safe and competent in the use of therapyequipment, relevant to the rotation setting, with support and training from thesenior therapist as appropriate.
To recommend/prescribe equipment and minor adaptations, following assessment and use ofclinical reasoning skills, in order to compensate for unresolved functionaldeficits, in line with local resources, guidelines and legislation.
Tofacilitate safe provision of equipment and instruct patients, family and carersin the use of equipment, as appropriate.
To assess,using clinical reasoning for minor adaptations, take measurements andcommunicate the recommendations, through reports and diagrams to adaptationproviders, with support from senior therapists.
To have anawareness of potential psychological risk factors and the functionalimplications of these.
To providespontaneous and planned specialist advice, teaching, and instruction torelatives, carers and other professions to promote understanding of the aims ofoccupational therapy and to ensure a consistent approach to patient care.
To beresponsible for equipment used in carrying out occupational therapy duties andto adhere to any relevant policies, including competence to use equipment andensure safe use of equipment by others through teaching, training andsupervision of practice.
Tosupervise assistants practitioner and support staff when appropriate
To provideclinical supervision for occupational therapy students on their firstintroductory placement and to explain the role of occupational therapy todepartmental visitors. Assist more senior staff in the training of students andoccupational therapy assistants.
Tocommunicate effectively with patients and carers to maximise rehabilitation potential and to ensureunderstanding of the their condition. Communication skills of persuasion,motivation and explanation will be used with a variety of patients.
Communicateeffectively with all members of the occupational therapy team, AHPs andcommunity nursing and other professions to ensure a collaborative approach tothe management of the patient's condition.
To assesscapacity, gain valid informed consent and have the ability to work within thelegal framework with patients who lack capacity to consent to treatment.
PROFESSIONAL:
To maintainown clinical professional development (CPD) by keeping abreast of any newtrends and developments and incorporate them as necessary into your work.
To be anactive member of the in-service training programme by attendance at trainingsessions in-house, Tutorials and by attending external courses and practicingreflective practice.
To maintainaccurate, comprehensive and up to date documentation in line with legal,departmental and ECCH requirements, communicating assessment and treatmentresults to the appropriate disciplines in the form of reports and letters.
Toparticipate in staff appraisal scheme and personal development plan (PDP).
To workwithin Organisational and Royal College of Occupational Therapists OTguidelines and adhere to the Professional Code of Conduct.
To have aworking knowledge of available equipment, minor adaptations and resourcesincluding Activities of Daily Living (ADL) equipment, therapy andrehabilitation aids, moving and handling equipment and a basic knowledge ofwheelchair provision.
Toundertake the measurement and evaluation of your work and current practicesthrough the use of evidence based practice (EBP) projects, audit and outcomemeasures, either individually or with other clinical staff and the clinicallead for occupational therapy. Make recommendations for change.
