The Role:

The OPPD Service supports older adults and adults with a physical disability. We are a multi-professional service comprising of Social Workers, Occupational Therapists, and Assistant Practitioners.

We are recruiting to an Assistant Practitioner post within our Assessment and Intake Team (A&I) and Community Review Team (CRT).


A&I provides a six week assessment and care planning service for services users based in the community and CRT provides a reviewing service for adults in receipt of services.


Within both teams you will be responsible for completing Care Act 2014 assessments to determine whether or not an individual is eligible for services.

This includes the undertaking of proportionate assessments, assessing ongoing support, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of some ongoing safeguarding work, under direct supervision from a qualified professional.


You will also be asked to complete Carers assessments to ensure that we are supporting the numerous informal carers within the borough.


About you:

We are looking for resilient practitioners who can manage competing demands and prioritise work effectively.

You will have experience of working with older people and people with disability and ability to undertake strength based social care assessments.

You will be a confident decision makers and who can stay calm under pressure. You will have experience of working effectively with other professionals and partner agencies as necessary.

You will have the ability to establish a rapport and build and maintain a professional, respectful and honest relationship with the service user and their family/carer.

You will have the ability to deliver focused interventions to safeguard vulnerable adults and promote independence, positive outcomes and promote the use of assistive technology.
