Assistant Practitioner - Band 4

Respiratory

Full time and part time roles available -Permanent role

Wiltshire Health and Care has an exciting opportunity for a Band 4 Assistant Practitioner to join the Wiltshire Community Respiratory Team.

This role will include working closely with respiratory physiotherapists to deliver a Pulmonary Rehabilitation service (exercise and education programme for people with Chronic Respiratory Disease) across Wiltshire; participate and lead on assessment of patients in preparation for pulmonary rehabilitation programme; support organisation and planning for the day to day running of the service; support data entry for audit purposes and provide clinical and administrative support with the wider team if required.
**This multidisciplinary team delivers a holistic patient centred approach, working collaboratively with Wiltshire Council Active Health programme, Community team services, Psychology service, Community Dieticians, Hospice support services. The team also provides the Wiltshire Home Oxygen Service, Respiratory Specialist Nurse caseload management, medication optimisation and exacerbation management.
