Description
This post offers an exciting opportunity to work within the award winning AIR service (Adult Integrated Respiratory Service). Ongoing support will be provided by the Respiratory MDT and two Band 7 Specialist Respiratory Physiotherapists.
The post holder will be responsible for the assessment of care needs, development, implementation and evaluation of care for people with respiratory conditions, in both the outpatient and domiciliary setting alongside the delivery of Pulmonary Rehabilitation across East Berkshire.
There will also be opportunity to contribute to the Home Oxygen Service - Assessment and Review (HOSAR), more specifically Ambulatory Oxygen Clinic and home visits to oxygen patients where necessary or as capacity dictates.
Close working within the MDT is essential, including assessment of patients for Early Discharge Service in the Acute sector and domiciliary work to review patients at home.

The active management of these individuals will support them and their families/ carers to develop self-management strategies with the specific aim of facilitating early supported discharge and preventing avoidable hospital admission.

If necessary, to be responsible for transferring individual patients on-going care to other health care professionals including senior respiratory physiotherapists within the AIR service and others in Primary and Secondary Car

This will allow you to provide high quality, responsive, respiratory physiotherapy services in clinics, patients homes or other appropriate locations
Work with the multi-disciplinary team, primary care teams and social services aiming to reduce avoidable hospital admissions and facilitate early discharge from the acute sector
To be responsible for prioritising and monitoring referrals to the AIR Team
To assess, plan, co-ordinate and deliver community based Pulmonary Rehabilitation programmes across all geographical locations in East Berks, with support from the senior physiotherapists
With training and supervision as needed, to contribute to the Ambulatory Oxygen Clinic alongside colleagues from the HOSAR giving specialist physiotherapy advice.

With training and support to use specialised physiotherapy manual skills, including dexterity, coordination and sensory skills in the assessment and treatment of patients.
Diploma/Degree in PhysiotherapyEvidence of CPD in a portfolio including attendance at recent post-graduate courses relevant to the clinical field/reflective practice in relevant clinical field
Formal post graduate qualification in a relevant area
Post graduate experience in respiratory physiotherapy/pulmonary rehabilitation
Supervision of juniors/assistants/students
Ability to demonstrate substantial relevant experience as a senior physiotherapist in the community/ward setting
Experience of service development/leading service development
Understand the legal responsibilities of the profession
Sound awareness of relevant National and local policies and guidelines relevant to the area
Ability to travel between multiple sites efficiently, car driver
Willing and able to work weekends and unsocial hours
