Job Duties:
An excellent opportunity has arisen in the Community Hospital Team for a team manager. If you are dynamic, like fast paced work, then we want to hear from you.

You will oversee a team of qualified and unqualified staff enabling them to arrange safe and sustainabledischarges, in partnership with key stakeholders.


The purpose of the role is to work in partnership with key stakeholders and to support staff to deliver, effective, timely, safe, and sustainable discharges, for the citizens of Hertfordshire.


If you have a proven track record of working in the hospital, delivering change, and effectively reducing and preventing delayed transfers of care, this is the job for you.

Responsibilities of the role include (but are not limited to);

  • Provide oversight and direction to staff (qualified and unqualified) within the Community Hospital team to deliver excellence in terms of practice and citizen engagement
  • Mentoring and coaching staff enabling them to develop their skills and knowledge to execute hospital discharges effectively and efficiently
  • Working with key stakeholders to reduce and prevent Delayed Transfers of Care.
  • Budget monitoring and management
  • Quality assurance of staff's assessment, reviews, care, and support plans. Ensuring that staff are utilising the Connected Lives model of strengthbased practice, enabling citizens of Hertfordshire to achieve their outcomes, and live fulfilled lives.
  • Recruitment, retention, and management of staff including professional development, and HR matters (where appropriate) e.g., performance and attendance management.
  • Oversight and allocation of referrals into the service matching complexity and risk with the level of skills and qualifications needed to undertake the necessary work.
  • Investigation and responses related to complaints
  • Taking ownership and oversight of the team ensuring we are delivering a safe and effective service, meeting the necessary Care Quality Commission's standards
  • Delivering change and supporting staff through change and transformation.
  • Building and sustaining effective relationships with key stakeholders
  • Report writing and delivery outlining the position pertaining to delayed transfers of care
  • Demonstrate leadership and implementation of strengths and evidencebased practice, to underpin decision making.
  • Been able to work as part of a team, as well as independently
The challenges of the role, include, delivery of change, reduction, and management of Delayed Transfers of care. Whilst balancing this with the connected lives approach, which enables Hertfordshire's citizens to achieve their outcomes and live fulfilledlives.


You will play a key role in taking the team forwards and building effective and sustainable relationships with key stakeholders and taking the team to the next step in terms of performance.

Its an excellent opportunity for an experienced leader or an experienced skilled practitioner wishing to take the nextstep in their career.

You need to be either a qualified Social Worker, or Occupational therapist, registered with your professional regulatory body.


It would be desirable for you to have qualifications in Practice Education, Best Interest Assessor or for you to be an Approved Mental Health Professional or have a qualification in management.

However, these are not essential.


You need to have a proven track record of delivering change, working in partnership with key stakeholders, hospital experience, and management and reduction of delayed transfers of care.


You will have excellent:
- communication skills
- case recording

  • IT Skills (Office 365, and Social Care data bases)
And you must be able to write and deliver reports to court standard.

Manage and oversee a budget, and have experience of working with key stakeholders e.g., Housing, NHS, and other key stakeholders.

You will need to be flexible in your approach, resilient, enabling you to respond to and meet demand.
