Upper Don NetworkClinical Pharmacist
We are looking for aClinical Pharmacist who will seek to improve the quality and safety of careoffered to patients of the Upper Don PCN. They will have regular contact withpatients and will reduce the workload of GP prescribers in the practice. Theywill also support the management of patients with long-term conditions
Main duties of the job
Specifically, thePractice-based Clinical Pharmacist will develop and manage medicinesoptimisation services within primary care. They will work as part of amultidisciplinary team to manage medicines on transfer of care and systems forsafer prescribing. They will perform face-to-face and telephone medicationreviews of patients with polypharmacy; especially for those with frailty and/orwith multiple co-morbidities. They will undertake face-to-face and telephonereviews of patients with specific long term conditions that fall within theircompetency (such as asthma reviews).

Working within yourprofessional and clinical boundaries as part of an establishedmulti-disciplinary team to provide expertise in all areas of medicine relatedpractice in a patient facing role.
Working inpartnership with various organisations, to improve the quality and safety ofcare offered to our patients. Using independent prescribing to support betteraccess to medicines, clinical medicines management, provide face to facestructured medication reviews, tackling polypharmacy and manage long termconditions.
To manage medicineson transfer of care and systems for safer prescribing including manage repeatprescriptions, acute prescription requests and antibiotic stewardship, whileaddressing both the public health and social care needs of patients across theprimary care network practices.
The post-holder isexpected to comply with Upper Don Network organisations employment policies andprocedures, as well as clinical and information governance policies andprocedures, and any other policies and procedures necessary for the safe andeffective discharge of the duties contained within this document.
The focus of thepost holders work will depend on the qualifications, skills and experience ofthe individual and the needs of the PCN Practice(s) which may develop over time.
