NMG are committedto developing a multidisciplinary diabetes team to support with the care ofpeople living with diabetes and those who have been newly diagnosed. We are looking for a pharmacist who has specialistexperience in diabetes to join an existing team to improve the care of peoplewith diabetes.
This exciting new role will giveyou the opportunity to develop your specialist clinical skills in apatient-facing, primary care environment, working with a wide variety ofhealthcare professionals.
You will need to understand thePractices strategic direction and work closely with the Practice team to develop and implement strategies to improveoutcomes for patients and the Practice, including service planning, monitoringand audit.
You will lead innovatively, based on clinicalevidence, to progress the Practice diabetes service including the wide use ofpopulation health and other relevant data to understand need and inform servicedevelopment, reducing or removing duplication and inefficiency. Implementingand evaluating the Integrated Diabetes services provision ensuring a smoothpathway through the service for patients with diabetes.

As a Pharmacist with special interest in diabetes you will:
Primary care experience would be an advantage. Excellent communication skills and interpersonal skills are essential plus being able to work as part of the practice team and autonomously.

Newgatemedical Group is a welcoming and busy GP Practice located in Worksop, Notts,providing regulated NHS services to our patients.
Weare a single site PCN with 30,000 registered patients. We have anestablished team of 9 GP Partners and a host of varied health professionals,employing over 80 staff, including pharmacists and 2 pharmacy technicians,assisting with medication reviews. Experienced and well developed nurse,HCA, management and admin teams, 3 trainee ACPs, PA, ANP & ACP undertakinghome visits and care home reviews. Also social prescriber link workers, CareCo-ordinators and excellent integration with community nurses and the voluntarysector.

Job descriptionJob responsibilities
Responsible for supporting the Partners in implementing effective medicine management within the practice, identifying areas for improvement and initiating and managing change, with a focus on supporting patients with diabetes.
1.Clinical
Develops, leads andcoordinates medicines optimisation services and a clinical pharmacy service forpeople living with diabetes within the PCN.
Enhances the existinggeneral practice/PCN diabetes team, by bringing a different skill set to thetable, to work collectively as part of the multi-disciplinary team to improvethe care of people living with diabetes.
Manages own clinical caseload of people living with diabetes,utilising prescribing qualifications to improve health outcomes, reducecomplications and reduce diabetes related hospital admissions.
Provides clinicalleadership to ensure high quality and cost-effective prescribing in diabetes,and other therapeutic areas.
Be an active member of andcontinue to build on the PCN DiAST team.
Leads on the implementationof new drugs and technologies for diabetes.
Provides highly specialisedpharmaceutical advice in complex situations (where advice may be challenged) tohealth care professionals, patients and carers..
Utilises advanced specialistknowledge covering a range of procedures and underpinned by relevantbroad-based knowledge, experience and competence.
Develops and maintains newskills in extended scope practice in response to emerging knowledge andtechniques.
Where appropriate toservice delivery, demonstrates highly developed physical skills which requirespecific training or considerable experience to attain the level required advanced moving and handling techniques, advanced clinical assessment, clinicalprocedures such as foot checks or diagnostic tests.
