    Main duties of the job

    Swansea Bay University Health Board is in the process of redesigning community services as part of the patient step up/step down pathways to maximise safe and effective patient care in their own homes. This will act to reduce avoidable hospital admissions and facilitate early patient discharge through the introduction of Cluster Virtual Wards. The virtual wards will have multi-professional input from Consultants, GPs, nurses and a wealth of other health and social care staff. The role of the Clinical Manager will be to co-ordinate the care of virtual ward patients through efficient leadership and co-ordination of the MDT and effective communication with patients, carers and colleagues across Primary Care, Community Services, Secondary Care and wider agencies.

    Detailed job description and main responsibilities

    The ideal candidate will be able to demonstrate through experience their ability to manage case loads of patients requiring complex care interventions through targeted assessment and treatment plans. They will be expected to take responsibility for the virtual ward patient register to ensure the right level of care is provided at the right time by the right clinician for our most frail patient population. The post holder will be able to drive change in a fast-paced environment, providing timely solutions through engagement with relevant community partners to ensure patient treatment is optimised and harm minimised. We are looking for applications from self-motivated, focussed clinicians who are effective communicators with a passion to provide current, evidenced based patient care.

    The successful candidate will join a well-established Cluster team and will have the opportunity to shape, develop and expand clinical pathways and models of care to directly benefit the patient population.

    Qualifications and/or Knowledge

    Essential criteria

  • First degree in associated professional role
  • Registration with relevant Professional Body (e.g. NMC, HCPC)
  • Education to Masters level or equivalent experience
  • Evidence of post registration education in a relevant field
  • Evidence of ongoing continuous professional development
