    This is an exciting time to join Herefordshire Council Children's and Young People's Directorate as we consolidate our improvement to shape learning and development across the service.

    We have a new and exciting vacancy for one full time Advanced Practitioner to join our Early Help Service who work to support families who have been assessed through an Early Help Assessment as we strive to achieve improved and sustainable outcomes for children, young people and their families.

    As an Advanced Practitioner you will be helping to ensure that the service works at the correct threshold and will be making threshold decisions to support this. You will have responsibility with the step-down process from social care, which will involve close liaison with the social care teams, reviewing case files in accordance with the Right Help Right Time document, developing appropriate wellbeing plans for children and families in line with our Supporting Families Outcomes Framework and ensuring a smooth transition via our mosaic system. You will take a lead role in reviewing referrals and making threshold decisions on these referrals.

    As an Advanced Practitioner you will have a crucial role in partnership working.You will represent the Early Help Service at various multi-agency panels and decision making forums which will include MARAC, Joint Decision Making Panel (Youth Justice Service), Children Missing in Education, amongst others. You will represent the Early Help Service confidently and professionally.

    You will also prepare and deliver multi-agency training with the aim of improving the quality of the Early Help Assessments and the Early Help Service that the children, young people and family receives.
