    Rehabilitation Support Worker - Primary Care Home- Waveney

    Hours of work hour and 12 hour shift

    We are looking for a part time Community Rehabilitation Worker to join our primary care home which is a multidisciplinary team of nurses, occupational therapists, physiotherapist, assistant practitioners, paramedic, health care assistants working closely with GP's, social services and other community providers to provide an excellent standard of person centered care to adults in their own homes, residential care or 'beds with care', supporting people to self manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.

    This role attracts paid enhancements for out of hours,bank holiday and weekend working.

    Main duties of the job

    As a support worker within the team you will be providing care and support to ensure the patients needs and goals are met, regularly liaising with team members to identify and report any changes in the patients condition. This includes daily rehabilitation programs with patients around but not limited to physical rehabilitation, personal care such as support with washing, dressing and toileting and meals.

    The Primary Care Home provides a 24/7 service and successful applicants will be required to work a variety of shifts including weekends and bank holidays.

    Previous experience of a caring role or community work would be an advantage however consideration will be given to the right candidate. The Care Certificate is required. If you do not already hold this certificate, you will be supported to complete the Care Certificate programme on commencing employment with East Coast.

    Job responsibilities

    Contribute to, support and promote ECCHs,strategic direction, values and culture in relation to primary care homeservices.

    Communicate effectively with patients,relatives and carers, respecting patient confidentiality with privacy andrespect for diverse cultural backgrounds and requirements.

    Communicate routine information to patients,relatives or carers, using skills of persuasion, reassurance, tact and empathyas required.

    To support relatives at all times and be awareof the effects of illness on the family.

    Enable individuals and their families toexpress their needs and preferences and take appropriate action to overcomebarriers to communication.

    To support and deliver aspects of activites ofdaily living. To include management of dietary and fluid intake, management ofcontinence, assisting with toileting programmes, including use of sheaths,catheters and continence pads.

    To promote confidence in the patient by meansof appropriate standards of appearance, behaviour and attitudes

    Check compliance with medication- Prompt,assist and manage medication as directed by Medication Administration Recordchart. Reporting any problems with medication and/ or compliance.

    To safeguard the confidentiality of allpersonally sensitive information.

    Contribute to the clinical governance agendathrough participation in clinical audit.

    Create an environment conducive to effectiveworking, respecting and supporting staff to deliver high quality clinicalservices.

    Work as part of the multidisciplinary PrimaryCare Home to deliver planned and unplanned services.

    Work with patients towards self-care andindependence.

    Participate in caring for patients at the endof life when appropriate.

    To implement the care as planned taking intoaccount own competencies.

    Take, record and report blood pressure,respiration, pulse rate, oxygen saturation levels, urinalysis. Be able torecognise and report the deteriorating condition of a patient

    To report any concerns / changes in thepatients condition to a registered nurse or appropriate Health care Professionalwithout delay.

    Undertake skin assessments. Undertake like forlike dressing change if required.

    Provide walking aids, provide and fit smallaids and equipment as directed.

    Ensure a high standard of record keeping isachieved in line with ECCH standards.

    Engage in personal development, maintaincompetence, knowledge and skills commensurate with role

    Take responsibility for the safe use ofclinical equipment,

    Contribute to the effective use of resources.

    Report anyconcerns/ changes in a patients condition toa registered health care professional without delay.
