Our client, a specialist homeless charity in Stoke-on-Trent is looking to recruit a Service Co-ordinator.


Synopsis of Duties:

  • Coordinate the work of 2 Link Workers in their role as Key Workers to people, ensuring that all people have an allocated Keyworker and up to date Risk Assessments and Support Plans.
  • Ensure delivery of quality services and strive for excellence.
  • Ensure that all staff receive Support and Supervision sessions, at least every six weeks in accordance with Policies and Procedures.
  • Review, develop and monitor the formal support coordination, support planning and risk assessment procedures.
  • Ensure that safeguarding procedures are followed and provide reports as required.
  • Write reports, monitor budgets and manage day to day expenditure of any service provision.
  • Ensure that all referrals are fairly dealt with and assessed as appropriate, and that communication is clear and comprehensive at each stage of the process.
  • Ensure that visits are arranged as per policy and procedure.
  • Liaise with external agencies including local statutory and voluntary services and ensure information is available to Link Workers and people relating to these services.
  • Raise awareness of services with relevant external agencies and in the community
  • Be aware of developments within the external environment in relation to current thinking and good practice in relevant areas of practice. Find ways to share these with partners and the team, e.g., training, seminars, written reports.
  • Assist in the recruitment and induction of new staff and volunteers.
  • Develop and maintain good communication with accommodation options to ensure all people receive proper advice and support with regards to appropriate accommodation.
  • Liaise with those promoting and providing meaningful occupation to ensure that Support Workers can promote the optimum level of activity for each person we support.
  • Provide reports as required to the Operations Manager and commissioners.
  • Take responsibility for health and safety practices within the office and your staff, ensuring that H&S checks are being carried out and records are being maintained in accordance with policies and procedures.
  • Ensure all H&S Risk Assessments are up to date and reflect the risks involved for the building and staff.
  • Regular monitor that staff are adhering to H&S policies and procedures and risk assessment management plans are being followed.
  • Participate in and ensure a lone working 'guardian' rota is active and being completed effectively by all staff to minimise risk for lone workers.
  • Participate in an oncall rota.
  • Be available on call to provide emergency advice & support to staff if required.
  • Take a lead role in evidencing Quality and Outcomes for the service as requested by Commissioners and internal audit team.
Essential Requirements

  • Managing a service including a staff team and budget in a support setting
  • Partnership and networking skills.
  • Knowledge and understanding of the principles of a traumainformed approach
  • Experience of working with people with a wide range of support needs including trauma.
  • Demonstrable experience of working with people who have complex trauma, which may include high mental health and/or substance misuse issues
  • Experience of delivering high quality outreach services to people with a wide range of needs and risks.
  • Ability to liaise and work closely with external professionals to inform and develop Service Users support
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately.

£19.00 per hour
