    Description

    Join our dynamic team at West Somerset Primary Care Network and step into a rewarding role covering surgeries in Minehead, Williton, Dunster, and Dulverton.


    You'll be based at the bustling Neighbourhood hub in Williton Community Hospital, alongside the Clinical Director, PCN Manager, and a dedicated healthcare team.


    As part of your role you will be directly involved in assessing patients referred from Primary Care (this will include adults of working age and older adults), followed by identifying and facilitating appropriate support/treatment for them.

    For most people this will mean remaining within Primary Care - supported either by the Primary Care Liaison for brief psychological interventions, the Open Mental Health Alliance or by Talking Therapies.

    For some, with more complex needs, it will involve facilitating a smooth transfer into Somerset Foundation Trust Secondary Mental Health services.


    Our healthcare team includes the PCN pharmacy team, social prescribing team (Care Coordinators, Village Agents and Health Coaches) and 'Living Better' complex care teams- including a Complex Care GP and a team of Complex Care Nurses and District Nurses.


    In this role, you'll play a vital part in guiding assessment, formulation, and treatment planning for individuals with mental health concerns.

    This includes developing comprehensive formulations and and assisting other practitioners in primary and secondary care to develop formulations as appropriate in relation to their caseload.

    You'll also facilitate liaison between primary and secondary care services, building relationships with GPs and other healthcare professionals.


    Additionally, you'll participate in multidisciplinary collaborations across both primary and secondary care settings and promote best practices in positive risk management.

    At West Somerset Primary Care Network, collaboration is key.

    We hold regular MDT meetings and streamline referrals through our single point of access, ensuring efficiency and seamless coordination of care.

    Our PCN also hosts frequent wellbeing and development days, prioritising the growth and support of our team members.

    To work collaboratively and flexibly with the multi-professional teams within Neighbourhoods to co-ordinate a coherent approach towards a mild to severe mental illness, actively promoting continuity of care and approach between services and to co-ordinate working practices.

    To ensure that there is adherence to local policies and practice.

    Expertise within all age mental health, including use of relevant assessments.

    Front line experience of using advanced assessment skills especially in determining the nature and severity of mental illness/health (functional) and differentiation between physical and mental health symptoms and conditions.

    Understanding of comorbidities that may affect older adults and their mental health
    Demonstrable experience of working in/ within CMHT or primary care mental health services
