    In response to the exciting changes happening in the community and Primary Care Networks (PCNs) we are recruiting Community Matron roles in The London Borough of Hammersmith & Fulham which is located in West London. For the right candidate, this is a real opportunity to shape patient care and put nursing right at the centre of community services.     Main duties of the job We are looking for a highly skilled Nurse with (or prepared to develop) advanced assessment and prescribing skills to join our well established team. You will work with our multidisciplinary Integrated Domiciliary Service (IDS) which utilises technology via virtual MDTs to improve communication and response time and enables clients to remain in their own homes. You will work closely with Community Services, Primary Care and the Voluntary Sector to provide case management for patients with complex needs. You will be responsible for setting high standards of practice and be a flexible and adaptable team player. You will have excellent communication skills and be able to motivate and engage both professional staff and clients.     • To co-ordinate care provision through a case management approach for a defined high risk patient group with complex long term conditions, such as COPD, diabetes, and CHD, with the aim to prevent hospital admissions and facilitate timely discharge.
    • To provide the highest standard of clinical care by using advanced skills and expert knowledge to holistically assess needs and instigate and provide clinical treatments based on evidence-based practice.
    • To participate in the delivery of educational programmes to patients, carers and health and social care workers that promotes self-care principles.
    • To develop and promote robust multidisciplinary working across the wider Health and Social care sectors to ensure integrated service and support networks for patients.
    • To monitor the quality-of-care provision and to identify and promote areas for service development. To be the named Community Matron for a Primary Care Network (PCN), providing support and care coordination for the wider health team within the network.
    • To provide clinical representation for the PCN at the Integrated Domiciliary Service (IDS) virtual MDTs
    • To work proactively with District Nursing colleagues and providing links across community nursing with the IDS.
    • To provide clinical leadership and support to end of life pathways, including the Coordinate My Care framework

    • Registered Nurse
    • First level Degree or equivalent
    • Relevant post-registration qualification (e.g. Specialist Community Practitioner)
    • Evidence of continuing professional development.
    • Commitment to study at Masters level.
    Desirable
    • Non-medical prescriber (willing to undertake)
    Physical Essential
    • To be occupational health cleared for the role specified
    Experience Essential
    • Post-registration experience, with experience at a senior level.
    • Extensive experience of assessing and providing care to people with complex long term conditions.
    • Experience of effectively working in collaboration, negotiating across organisational boundaries.
    • Experience of change management and experience of teaching others
    • Experience of receiving and delivering clinical supervision
    • Experience of providing evidence-based care
    • Experience of audit and evaluation
    • Community experience

    • Experience of report writing
    • Experience of policy development
    Knowledge Essential
    • Extensive bio-medical knowledge across a range of long term conditions
    • Detailed knowledge of Primary Health Care and political influences
    • Knowledge of research / audit processes and knowledge of clinical governance and its implications
    • Knowledge of adult safeguarding issues
    • An understanding of the Comprehensive assessments
    • Knowledge of risk management issues and knowledge of resource management
    • Outstanding quality outcomes measures and relevant to service delivery
