We have an exciting opportunity for one person to join our Children and Young People's Emotional Wellbeing & Mental Health Service as a Community Connector in a social prescribing pilot initiative.

This role will help children and young people to work on their wider health and wellbeing through active involvement with activities and groups in their local community - particularly in response to the ongoing impact of the Covid-19 pandemic on young people's wellbeing.


You will play a key role in delivering team outcomes by working directly with children and young people referred to our Single Point of Access to provide them with access to community activities alongside or instead of traditional mental health support.

You will also support the Project Lead in promoting, recording and evaluating the outcomes of the social prescribing pilot.


You should have experience of engaging with and empowering people, including children and young people, who are facing complex social and emotional challenges.

You will be able to build trusting relationships with children, young people and families, co-produce personalised care plans and connect individuals to activities and groups that will help them to achieve their goals.

You will be confident in building and maintaining relationships with people working across the third sector and public sector.

This post is fixed term until 31stMarch 2024 to meet the needs of the service.

  • Deliver one to one 'What matters' conversations with children and young people to determine what's important to them, their goals and coproduce a plan to achieve these
  • Connect with internal stakeholders such as the Single Point of Access and School In Reach team as part of coproducing personalised care plans
  • Be able to build trusting relationships and networks with third sector agencies
  • Support the ongoing audit and evaluation of the social prescribing pilot
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
