Are you a Registered Nurse (Child) or Registered (Learning Disability) who is passionate about providing an excellent experience to Children and Young People (CYP) with Special Educational Needs/Disability (SEND) and physical disability?


We are looking for friendly, dynamic and proactive nurses to join our established team helping CYP with SEND and physical disability navigate services during their transition from children's to adult services with a focus on providing high quality care.


The successful applicant will possess excellent communication skills and support young people from the age of 14 to 21 maximizing their opportunities for independence and choice.


The role of the PFA Co-Ordinator is primarily concerned with providing a high quality health care experience for Children & Young People (CYP) with Special Educational Needs / Disability (SEND) or physical disability whilst ensuring a safe, smooth and seamless transition to adult services.

Paying special attention to the SEND Code of Practice the job holder will work to provide a key worker for 14-21 year old's that meet referral criteria.

An important element of the role will be to ensure that the multiple services/agencies often involved in the care of the CYP are joined up and communicating effectively with the CYP's voice being at the heart of decision making.
