We are delighted to offer this opportunity for a part time post to work as part of the award winning Macmillan End of Life Care Facilitation Team to provide facilitation and programme management to the implementation of tools and training required to support high quality end of life care across BCUHB.


The post holder will be part of North Wales Department of Palliative Care and is responsible for the implementation of the strategic vision of end of life care in Nursing Homes, Community Hospitals, primary care and secondary care as directed by leads for end of life care.

The post holder will work pre-dominantly in North East Wales area but will also be required to work flexibly across North Wales when required.


They will be responsible for the co ordination and delivery of the work and education streams that form the strategy, improving both the patient experience and efficiency of advance individual care planning around this specific area.


The post holder will provide programme management to support end of life care, facilitating delivery on key aspects of implementation, development and evaluation of the strategic work streams.


The post holder will provide advice and challenge in order to ensure that the vision and ethos of the Strategy is adhered to.

They will assist with the developing, co-ordinating and embedding of clear processes for the delivery of the strategic work streams along with performance management and reporting of strategy progress.

Informal enquiries and visits are strongly encouraged.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
