Thepartnership is responsible for Montpelier Health Centre, a PMS Practice, ininner city Bristol which has a diverse population of just over 20500 patients. The CQC rating is Good.
We work outof two buildings on the same site, one of which we own which is modern and wellmaintained, with Montpelier Health Centre being leased from NHSPS.
Thepractice works collaboratively within their Primary Care Network, Bristol andInner City PCN (BIC).

Main duties of the job
The main clinicalduties are to:
Be available toundertake a variety of duties including surgery consultations, telephoneconsultations, visiting patients at home and in nursing homes, checking andsigning repeat prescriptions and dealing with queries, paperwork andcorrespondence as required.
Make professional,autonomous decisions in relation to presenting problems, whether self-referredor referred from other health care workers within the organisation
Screen patients fordisease risk factors and early signs of illness
Cover all theclinical management of the patients in surgery as appropriate includinginitiating investigations, reviewing results, making referrals to secondarycare or other providers as appropriate
Develop care andtreatment plans in consultation with patients and in line with current Practicedisease management protocols as well as wider guidance such as that from NICE
Ensure appropriatefollow-up of patients
Record clear andcontemporaneous computerised consultation notes to agreed standards
Collect data foraudit purposes
Prescribe inaccordance with the Practice Prescribing Formulary, NICE guidance and GoodPrescribing Practice (as defined by British National Formulary guidance)
Work with andsupport other members of the primary health care team in delivering high-quality primary care
Play a lead role inthe practice based on strengths and interests.
