    Job Description

    The position of Substance Misuse Practitioner presents a fantastic opportunity to become part of a proactive team of professionals dedicated to assisting and empowering at-risk children and young individuals. In this role, you will deliver specialized interventions to a caseload of young people by conducting thorough assessments, developing care plans, and managing risks effectively.

    Main Responsibilities

    • Developing, monitoring, evaluating, and reporting on care plans and risk assessments, ensuring appropriate support for individuals undergoing treatment.
    • Offering family interventions and assisting parents, carers, and potential service users.
    • Promoting service user involvement through group activities, facilitating volunteers, and engaging with the community.
    • Collaborating with different agencies to establish effective communication, referrals, and care pathways.
    • Partnering with the criminal justice system to help young people address substance misuse issues and prevent re-offending.
    • Conducting harm reduction initiatives, providing drugs education, and supporting individuals and groups within educational settings.
