About the role

We are the front door for these communities so If you are passionate about working with people to promote independence and dedicated to making a difference, then we would love to hear from you.

We are looking for enthusiastic and dynamic person who will become part of the local community and work directly with people undertaking conversations with people, signposting, and supporting them to access their community and completing Care Act assessments of people to identify any needs.


West Northants Council uses daily a model, three conversations, that re-emphasises focus on people, their strengths, and the networks through the development of community and Inclusion hubs.

We are looking for staff who believe it is important to ensure people have choice and control over their own support needs and focus on the person, their strengths, and networks.


You will work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments and people with mental health needs.

About you
Undertake clear and accurate assessments and reviews of an individual in line with statutory guidance and best practice.


Provide short term intervention plans for individuals as appropriate, ensuring individuals are supported and encouraged to access community support that is available.

Effectively assess and manage risk, for example safeguarding and take appropriate action when required.


Use a person centred approach, listening and developing creative and personalised solutions to assist people to manage their lives independently for as long as possible.

Provide accurate, impartial information, support and guidance to customers and their families.
