- This role involves collaborating closely with various teams, including the Community Resource Team and the Information, Advice and Assistance (IAA) team.

- The service operates 7 days a week, all year round, between 07:30 and 20:00, requiring flexibility in working hours.

Main Duties:
Join the collaborative effort between Aneurin Bevan Health Board and Blaenau Gwent Social Services to provide timely interventions to adults requiring support to stay in their homes.
You will work alongside various professionals within the multidisciplinary team, such as Social Workers, Occupational Therapists, Physiotherapists, and more.
