East Suffolk Primary Care Network (ESPCN) is seeking twodedicated and experienced Cancer Care Nurses. One post will cover mid-Suffolk(Eye to Stowmarket) for 34 hours per week and the other Ipswich & Felixstowe for 30 hours per week.

ESPCN is a collaborative network of GP practice in EastSuffolk. We work together along with community, mental health, social care, pharmacy, hospital, and voluntaryservices to provide integrated health and care services to our 130,000patients. The role will be employed by Suffolk Primary Care, an at-scaleprimary care provider covering 8 practices in East and West Suffolk.
Main duties of the job
Your Role
As a PCN Cancer Care Nurse, you'll play a pivotal role in driving forward our cancer care agenda. Your responsibilities will include:
Conducting Cancer Care Reviews for our patients, ensuring the very best personalised care and support.
Supporting 2WW suspected cancer pathway referrals into secondary care, ensuring all pre-referral tests are completed.
Scoping community services to aid in early cancer identification and linking into local communities and voluntary sector organisations.
Leading the development of health promotion pathways and driving improvements in the quality of Cancer Care Reviews in primary care post-diagnosis.

Job responsibilities
Job Summary
As part of East Suffolk PCN, your role is to develop andimplement a comprehensive cancer pathway approach, aligning with the NHS LongTerm Plan's goal of earlier cancer diagnosis.
You will collaborate closely with system partners to ensureseamless coordination of care for patients on cancer pathways. This includesfacilitating timely referrals, providing specialist advice, and supportingpatients living with and beyond cancer to enhance their quality of life.
You will forge relationships with community services,secondary care, and PCN practices to ensure early symptom identification,supported referrals, and robust safety measures. Additionally, you will leadeducational initiatives, contribute to multi-professional healthcare teamtraining, and drive service development through clinical audit and leadership.
By working collaboratively with system colleagues, you willaddress variations and inequities in cancer care, ultimately improving patientpathways across primary, community, and secondary care settings.
Scope of the Role
To support the 2WW suspected cancer pathwayreferrals into secondary care, by ensuring all pre referral tests arecompleted. Working within an agreed framework that includes safety netting andreturning to the referrer.
To scope the community services available thatsupport early identification of cancer, there are some health inequalitieswithin identifying early cancer at an early stage. This role will link intolocal communities and Voluntary sector organisations within the local area tosupport education and awareness.
To be the PCN point of contact for cancer insupporting other practice nurses and administration staff to identify thesupport patients may require that are having cancer treatments and living withand beyond cancer within the PCN.
Lead on the development of a Make Every ContactCount (MECC) through embedding a health promotion pathway for those patientswho have been referred on a cancer pathway and are discharged followingsecondary assessment. Those patients who undertake risk taking behaviour suchas smoking or excess alcohol may be our future cancer patients.
To lead improvements in the quality of CancerCare Reviews (CCR) completed in primary care post diagnosis.
To support the development of innovations acrossall aspects of the cancer pathway.
Clinical Responsibilities
Be actively engaged in the setting up andestablishment of this new service, actively engaging with other stakeholders toensure that processes are robust and the service operates within the agreedguidance.
Be responsible for coordinating the pre referraltests required for 2WW referrals within the PCNs, interpret results of relevantpatient investigations in conjunction with clinicians and where indicatedprovide appropriate management as directed by the clinical team with next stepswithin the protocol led pathway.
Be responsible for working with the referring GPfor safety netting patients who are on a cancer pathway until point ofdiagnosis or discharge.
To complete and maintain accurate records ofpatient interactions, pre-diagnostic/ referral interventions clinical narrativeand outcomes, ensuring that patient confidentiality is adhered to within thePCN Information Governance guidelines.
To undertake holistic needs assessments ofidentified patients, enabling timely support and advocacy of patientspersonalised care needs providing relevant information and psychologicalsupport; advising patients and their identified carers of expected next steps.
Actively monitor and escalate issues regardingthe patient referral and next steps to the responsible clinician.
Develop and participate in the promotion ofpatient-centred care and establish and maintain a supportive relationship withthe patient and their family, in making informed decisions about their carefrom the point of suspected cancer referral.
Develop a continued updated knowledge of thecurrent services available for patients with complications from their treatmentbut also services to offer on-going support in primary care into survivorship.
Developing the service to include a regular faceto face clinic for patients with higher support needs who are having treatmentor are on a personalised follow up pathway.
