    Job summary

    We are seeking a Senior Staff Nurse or Hospice @ Home Paramedic to join our team to provide individualised holistic care to patients across eastern Hertfordshire.

    This role will work closely with the Isabel Hospice Community Specialist Team and In Patient Unit and will provide cover to In Patient Unit services if required and includes weekend working. You will work with other health/social care professional in the community, hospitals, and Acute Trust to ensure that patients requiring end of life care receive care in an integrated, timely manner and are referred to other parts of the Isabel Hospice service as required.

    These patients will have a palliative diagnosis such as dementia, cancer, organ failure and neurological conditions and will have been referred to our services by fellow health care professionals, the patient or family.

    Main duties of the job

    To apply you should be a Level 1 registered Nurse or registered Paramedic with a minimum of two years post registration experience, preferably within the community. Experience of palliative/end of life care would be desirable.

    We offer ongoing membership of the NHS pension scheme for existing members and honour long service leave gained at the NHS or membership of our company pension and life assurance scheme. We also offer free parking, tea & coffee, Employee Assistance Programme, life assurance and pension. Our enhancements for unsocial hours are 30% after 8pm weekdays and all day Saturday, 60% all day Sunday.

    Job responsibilities

    To deliver high quality holistic care to any patients with a terminal illness such as Dementia, Cancer, Organ Failure (end stages) and Neurological conditions within the community setting . These patients should have been referred to our services by fellow health care professionals or the patient or to Day Operations

    Assess, plan, implement and evaluate pathways of care for patients in a way that reflects the Hospice philosophy.

    Providing a rapid and coordinated response to prevent unnecessary hospital admissions at the end of life.

    Support patients to achieve their end of life wishes to die in the preferred place of care.

    Agree individualised patient centered care plans to deliver a high standard of evidence based palliative care working in partnership with external agencies where necessary.

    Review, deliver and update care plans and documentation, escalating all concerns to the appropriate individual.

    Recognise and react to all palliative care emergencies appropriately.

    Work alongside other members of the team in assisting with teaching patients and their family/carers in preparation for changes in treatment and expectations.

    Promote a safe environment for all patients, carers, visitors and staff in the patients own home as appropriate.

    Develop an understanding of available resources in the community which will facilitate and support the provision of care to the patient requiring specialist palliative care.

    Communicate effectively with members of the wider team.

    Safely and competently administer all medications including subcutaneous medications as applicable.

    Be able to work independently within the parameters of own competence.

    Provide a high standard of sensitive communication with patients, families and carers, which maintains an individuals privacy and dignity.
