Description

About The Role:

An exciting opportunity is available for an enthusiastic, committed and creative person to join the Adult Access Team as an Assessment and Support Co-ordinator.

The Adult Access Team is the 'front door' to adult social care for new and returning customers in the Dorset Council area and is based in County Hall, Dorchester.

Using a strengths-based approach, we work with our customers to understand the outcomes they are looking to achieve, their strengths and assets and what support they may need to achieve their goals.

We aim to resolve as many enquiries as we can at the first point of contact by linking people with their local communities, providing information and advice, issuing small pieces of equipment which promote independence, considering what assistive technology may be helpful and if further assessment is required, referring to our colleagues in the locality teams.

This is a fixed term position until 31st March 2025
