We would like to extend an invitation to join our well established Urology CNS team, as a Band 6, if you feel you have the right skills and qualities.
To work clinically as a urology nurse specialist providing a range of nurse led clinics, support the service and support the department to develop your knowledge and skills.
Identifying and directing patient referrals from the GP and other sources to the most appropriate clinic. To provide a high standard of evidenced based care for patients with urological conditions.To provide teaching and develop clinical expertise across the hospital sites and community teams with a passion for improving quality.

To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
To undertake nurse led clinics and to be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics.
To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
To contribute to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients.
To contribute to training and education within specialist area.To promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities.

Through the principles of the Best Practice Prostate Pathway, it is anticipated that urology patients on these pathways will have an improved waiting time to diagnosis with a positive patient experience.
The CNS will also work along site Consultants and support the Urology cancer and benign services.The CNS will develop to work effectively as an autonomous practitioner and as a core member of the multi-disciplinary team within Urology to provide the highest possible standards of care for patients with urological disease.
The CNS will work with the support and direction of the head of nursing for surgical specialties, the matron, the Trust lead cancer nurse and the Consultant urologists to improve the care of patients, provide specialist help advice and support to patients and their families.
The post holder will develop and work as an integral member of the Multidisciplinary Team across the Worcestershire Acute Hospitals, collaborating with a range of health care providers.
The post holder will provide highly developed specialist nursing, Education, Research and Audit, Leadership and Consultancy and a Professional Role model.
Will develop to provide specialist nursing practice intervention so that the specific needs of individual patients / clients are addressed.
Provide direct care as per job plan, working alongside the nursing staff in the clinical area- providing a visible and strong role model to ensure best practiceWill develop to conduct independent nurse led clinics for a variety of urological conditions that need monitoring/treatment or review post-surgery.
To provide advice and support for all members of the multi- professional team in the care of patients undergoing diagnostic pathway with suspected urological cancer.
To liaise closely with members of the multi professional team so that patients with confirmed urology cancer experience a seamless, high quality service.
To provide guidance, support, advice and counselling to patients and their family/ carers along the patients pathway. To know when to refer on to other professionals for more specialist guidance, support, advice and counselling.To promote patients autonomy through education and support so that the individual is able to make informed choices about their own care.
To recognise opportunities within the urology service for further development and expansion, to facilitate and manage change within defined clinical and organisational boundaries.
To develop effective working relationships with all other departments resulting in effective coordinated care for clients, with all other local, regional and national organisationsTo set, monitor and review standards on a regular basis in accordance with clinical governance essence of care benchmarks and the NHS plan.
To liaise with MDT and Matron re:
leading on patients satisfaction surveys and present findings at Clinical Governance and other appropriate meetings.
Develop skills that enable the CNS to lead on developing integrated care pathways and nursing care pathways in liaison with members of the multidisciplinary team.
To assist in the development own nurse led clinics in conjunction with the Consultants and the outpatients department, after consultation with the Matron and key stakeholders.
To maintain nurse led and telephone clinics whereby patients are provided with information and support and the problems and needs of patients and their careers are assessed and appropriately channelled and referred to other services as required.
To actively teach the nursing staff in the clinical area and evaluate their learning in conjunction with the senior nursing staffTo work collaboratively on educational strategies which support urology cancer services and service development
To develop and deliver education programmes on the ward in liaison with the multidisciplinary team/nursing staff.
Develop their knowledge, skills and expertise to a higher academic standard.
To have a good understanding of the value of quality research to improve patient care
To utilise up to date research/evidence to deliver patients care and to disseminate this knowledge to all members of the multi-professional team
To audit the service on a regular basis so that patients care is continuously reviewed and improved, ensuring that documentation and record keeping is comprehensively maintained.
Assist in the engagement with both internal and external groups which will lead to the further development of the nursing service and the Clinical Nurse Specialist role.
To monitor and evaluate ones own service implementing new initiatives as appropriate to improve patient care.Contribute to the development of clinical services across the trust, through liaison with the Head of Nursing and other specialties.
To work with and alongside local and nation networks.Work closely with other nurses to assist with their own professional development.
To act in accordance with the NMC Standards for conduct, performance and ethics for nurses and midwifes and to be accountable for his/her actions at all times.
To adhere to the Data Protection Act, maintaining and safeguarding Patients confidentiality at all times and only releasing information to those acting in an official capacity.
